陈竹教授从伏邪论治小儿支气管哮喘

Professor CHEN Zhu’s treatment for children with bronchial asthma based on syndrome differentiation of latent pathogenic factors

陈竹教授基于伏邪理论,认为伏痰阻肺是小儿支气管哮喘发病的根本原因,且伏痰常与热、风、瘀胶着为患,急性发作期多为痰热蕴肺证,治以泻热宣肺、化痰平喘,方以清气化痰汤加减。慢性持续期为痰热恋肺、虚实夹杂证,治以清肺化痰、健脾助运。临床缓解期多见肺脾两虚证,治以健脾补肺固本,但患儿表现不一,故均予自拟方加减调理。陈教授强调治疗该病应遵从“急则治其标,缓则治其本”的原则,且理气、祛痰、化瘀应贯穿治疗始终。附案例1则,以资验证。

Professor CHEN Zhu holds that latent phlegm obstructing lung is the underlying cause of the onset of bronchial asthma in children,moreover,latent phlegm often closely combines with heat,wind and blood stasis to induce disease.Phlegm heat accumulating in lung syndrome is seen mostly during the acute attack period,so it is advised to treat with purging heat and dispersing lung as well as resolving phlegm to resolve dyspnea,selecting prescription modified Qingqi Huatan Tang(清气化痰汤).Phlegm heat lingering in lung syndrome and deficiency mingling with excess is seen during the chronic persistent period and the treatment is clearing lung to resolve phlegm and strengthening spleen to assist transportation.Asthenia in both lung and spleen syndrome is seen mostly during clinical remission period,and the treatment is invigorating spleen to tonify lung and consolidate vital base.However,the clinical manifestations of children are different,so they are treated with modified self-made prescriptions for regulating.Professor CHEN stresses that the treatment of the disease should follow the principle of“relieving secondary symptoms in acute cases and treatment of chronic disease focusing on the underlying cause”,and regulating qi,expelling phlegm and removing blood stasis should span the whole course of treatment.One case is attached for verification.
