DWI成像联合3D剪影技术对宫颈癌诊断及分期的价值

摘要 目的:探讨磁共振(MRI)检查弥散加权(DWI)成像联合三维(3D)剪影技术在宫颈癌的诊断及影像分期的临床价值。方法:收集经病理证实的36例的宫颈癌患者临床及MRI资料,所有患者均行常规扫描外,还行DWI(b=100、500、1000s/mm^(2))、动态增强扫描,并进行3D剪影技术,对此进行影像诊断及术前分期,将术前影像分期与临床分期结果进行对比、分析,以评估DWI成像联合3D剪影技术对宫颈癌诊断及分期的价值。结果:宫颈癌在T1WI上呈等信号或略低信号,T2W上呈高信号,DWI呈高信号(随b值升高,信号增高),增强后动脉期肿瘤强化明显,门脉期及延迟期强化减退;36例病灶均能清晰显示,MRI对宫颈癌的分期准确率到94%;3D剪影图像能更好显示病变的形态、强化特点、与周围组织关系及淋巴结转移情况。结论:MRI检查是诊断宫颈癌的重要方法,DWI成像联合3D剪影技术能提高宫颈癌诊断及分期的准确性,为临床治疗方案的制定提供重要的依据。 Objective:To investigate the clinical value of DWI imaging combined with 3D silhouette in the diagnosis and imaging staging of cervical cancer.Methods:collecting 36 cases of cervical cancer,which was confirmed by pathology,clinical and MRI data of all patients were performed regular scanning,DWI(b=100,500,1000s/mm^(2)),dynamic enhanced scan,and 3D silhouette technology,and for image diagnosis and preoperative staging,comparing with the results the preoperative imaging staging and clinical staging,analysis,to assess the DWI imaging can joint3D silhouette technology to the value of diagnosis and staging of cervical cancer.Results:Cervical cancer showed isosignal or slightly low signal on T1WI,high signal on T2W,and high signal on DWI(signal increased with the increase of b value).After enhancement,tumor enhancement was obvious in arterial phase,and decreased in portal phase and delayed phase.The lesions in 36 cases could be clearly displayed.3D silhouette image could better display the morphology,enhancement characteristics,relationship with surrounding tissues and lymph node metastasis.The accuracy of MRI in cervical cancer staging was 94%.Conclusion:MRI examination is an important method for the diagnosis of cervical cancer.DWI imaging combined with 3D silhouette technology can improve the accuracy of diagnosis and staging of cervical cancer,and provide an important basis for the formulation of clinical treatment plan.
