211例三、四期梅尼埃病患者前庭功能检查与眩晕残障程度评定量表评分的相关性

Correlation between vestibular function and DHI scores in 211 patients with advanced Meniere's disease

目的探讨三、四期梅尼埃病(Meniere's disease,MD)患者前庭功能检查与眩晕残障程度评定量表(dizziness handicap inventory,DHI)评分的相关性c方法对211例三、四期MD患者行前庭功能检查和DHI评分,检查包括冷热试验(caloric test,CT)、颈肌前庭诱发肌源性电位(cervical vestibular evoked myogenic potentials,cVEMP)和眼肌前庭诱发肌源性电位(ocular vestibular evoked myogenic potentials,oVEMP)。分析各项检查的异常率,并分析检查参数与DHI评分的相关性。结果三、四期MD患者冷热试验和cVEMP检查的异常率明显高于oVEMPc患耳冷热试验的CP值、cVEMP阈值以及oVEMP阈值与DHI量表得分均呈正相关。结论三、四期MD患者的水平半规管和球囊功能下降最明显,前庭功能检查主要参数和症状明显相关。

Objective To explore the correlation between vestibular function and dizziness handicap inventory(DHI)scores in patients with advanced Meniere's disease(MD).Methods Vestibular function tests and DHI scores were performed on 211 patients with advanced Meniere's disease,including caloric test(CT)and cervical vestibular evoked myogenic potentials(cVEMP),Ocular vestibular evoked myogenic potentials(oVEMP).The correlation between the parameters of the vestibular function tests and DHI score was analyzed.Results The abnormal rate of CT and cVEMP in patients with advanced Meniere's disease was significantly higher than that of oVEMP.The CP value of caloric test,cVEMP threshold,oVEMP threshold and DHI score in patients with advaneed Meniere's disease were positively correlated.Conclusions In patients with advanced Meniere's disease,the function of horizontal semicircular canal and saccule decline most obviously,and the main parameters of vestibular function and symptoms are obviously related.
