自制中药膏外敷联合手法复位小夹板外固定对桡骨远端骨折患者骨折愈合及腕关节功能的影响

The influences of TCM ointment external application plus manipulative reduction and small splint external fixation on fracture healing and wrist joint function of distal radius fractures patients

目的:探讨分析自制中药膏外敷联合手法复位小夹板外固定对桡骨远端骨折患者骨折愈合及腕关节功能的影响。方法:选取2018年5月-2020年5月收治的138例桡骨远端骨折患者,分为观察组和对照组,对照组给予手法复位小夹板外固定治疗,观察组联合自制中药膏外敷治疗,比较两组临床疗效、腕关节功能恢复情况、治疗前后腕关节活动度及Cooney量表评分。结果:观察组消肿时间、骨折愈合时间较对照组显著缩短(P<0.05),术前两组腕关节Cooney评分无显著性差异(P>0.05),术后1个月观察组Cooney评分提高更显著(P<0.05),观察组腕关节功能恢复优良率较对照组显著提高(P<0.05),临床疗效总有效率较对照组显著提高(P<0.05),治疗前两组腕关节活动度差异无统计学意义(P>0.05),治疗后观察组背伸、掌屈、尺偏、桡偏、旋前、旋后活动度较对照组提高更显著(P<0.05)。结论:手法复位小夹板固定联合自制中药膏外敷治疗桡骨远端骨折可显著提高临床疗效,促进消肿和骨折愈合,恢复腕关节功能,值得推广。

Objective:To investigate and analyze the influences of traditional Chinese medicine ointment external application plus manipulative reduction and small splint external fixation on fracture healing and wrist joint function of distal radius fractures patients.Methods:138 cases from May 2018 to May 2020 were divided into the observation group and the control group.The control group was treated by manipulative reduction and small splint external fixation,and the observation group was given self-made traditional Chinese medicine ointment external application more.Results:Compared with the control group,the swelling disappear time and fracture healing time in the observation group was significantly shorter(P<0.05).The Cooney score in the observation group was improved more significantly at 1 month after surgery(P<0.05).The excellent-good rate of recovery of wrist joint function in the observation group was significantly improved(P<0.05).The total efficiency in the observation group was significantly improved compared with the control group(P<0.05).After treatment in the observation group,the motion range of dorsiflexion,palmar flexion,ulnar deviation,radial deviation,pronation,and supination was increased significantly compared with the control group(P<0.05).Conclusion:Manipulative reduction plus small splint external fixation and self-made traditional Chinese medicine ointment external application on distal radius fractures can significantly improve clinical efficacy,promote swelling disappear and fracture healing,and restore wrist joint function of patients.It is worthy of promotion.
