探讨重组组织型纤溶酶原激活剂静脉溶栓治疗不同年龄段急性脑梗死的临床疗效分析

Investigation on the Clinical Efficacy of Recombinant Tissue Plasminogen Activator Intravenous Thrombolysis in Acute Cerebral Infarction at Different Ages

目的探讨急性脑梗死不同年龄段采用重组组织型纤溶酶原激活物(rt-PA)静脉溶栓治疗的效果。方法回顾性分析2017年5月—2018年5月该院收治的200例不同年龄段的急性脑梗死患者作为该次研究对象,将患者按照不同年龄段分成50~60岁组,60~70岁组,70~80岁组以及80岁以上组,每组各50例患者。比较治疗后所有组别的疗效,神经功能缺损(NIHSS)评分,神经功能恢复情况(mRS)评分,出血情况。结果4组患者治疗后的疗效、mRS评分、出血量比较,差异有统计学意义(P<0.05),NIHSS评分均低于治疗前,差异有统计学意义(P<0.05)。其中50~60岁组患者的总有效率为90.00%,60~70岁组的72.00%,70~80岁组的68.00%,80岁以上组的58.00%,差异有统计学意义(χ^(2)=13.294,P<0.05);50~60岁组的NIHSS评分,mRS评分,出血率均低于70~80岁组以及80岁以上组,差异有统计学意义(P<0.05)。结论对于急性脑梗死不同年龄段的患者都可以在静脉溶栓中获益,而患者年龄较低的其获益更高,预后会更加理想。

Objective To investigate the effect of intravenous thrombolytic therapy with recombinant tissue plasminogen activator(rt-PA)in different age groups of acute cerebral infarction.Methods From May 2017 to May 2018,200 patients with acute cerebral infarction of different ages admitted to the hospital were retrospectively analyzed as the research objects.These 200 patients were divided into 50-60 years old group,60-70 years old group,70-80 years old group and over 80 years old group according to different age groups,with 50 patients in each group.The therapeutic effect,neurological deficit(NIHSS)score,neurological recovery(mRS)score and bleeding were compared in all groups after treatment.Results There were statistically significant differences in the curative effect,mRS score and blood loss between the 4 groups after treatment(P<0.05),and NIHSS score were all lower than before treatment,the differences were statistically significant(P<0.05).The total effective rate of the 50-60 year old group was 90.00%,higher than that of the 60-70 year old group 72.00%,68.00% in 70-80 years old group and 58.00% in over 80 years old group,the difference was statistically significant(χ^(2)=13.294,P<0.05);NIHSS score,mRS score and bleeding rate in 50-60 years old group were lower than those in 70-80 years old group and over 80 years old group,the difference was statistically significant(P<0.05).Conclusion Patients of different ages with acute cerebral infarction can benefit from intravenous thrombolysis,while younger patients have higher benefits and better prognosis.
