无创产钳在阴道助产中的临床应用 被引量:1

摘要 目的探讨无创产钳在阴道助产技术中的应用效果。方法便利选取2020年5月—2021年5月在该院进行阴道分娩中需要阴道助产的初产妇80例,按照数字表法随机分组,40例为对照组,40例为研究组。对照组应用会阴侧切产钳助产,研究组应用无创产钳助产,对两组产妇结局、新生儿结局、产时出血量、产后会阴痛、产后42 d盆底功能进行观察与记录,并比较两组观察结果。结果研究组产妇并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。研究组与对照组新生儿并发症发生率差异无统计学意义(P>0.05)。研究组产时出血量为(202.27±11.62)mL,产后会阴痛评分为(2.35±0.72)分,明显少于对照组的(302.83±13.01)mL,(3.81±0.76)分,差异有统计学意义(t=36.460、8.820,P<0.05)。研究组产后42 d阴道动态压力值、Ⅰ类盆底肌力、Ⅱ类盆底肌力、Ⅰ类肌纤维疲劳度、Ⅱ类肌纤维疲劳度分别为(85.32±3.56)cmH_(2)O、(3.46±0.42)级、(3.42±0.44)级、(-0.04±0.01)%、(0.03±0.01)%,与对照组(81.18±3.71)cmH_(2)O、(2.52±0.41)级、(2.54±0.43)级、(-0.06±0.01)%、(0.05±0.02)%比较,差异有统计学意义(t=5.092、10.129、9.046、8.944、5.657,P<0.05)。结论在阴道助产技术中应用无创产钳的临床价值非常高,有助于改善产妇结局及盆底功能,同时还可以降低产时出血量及产后会阴痛,值得临床推荐与应用。 Objective To explore the application effect of non-invasive forceps in vaginal midwifery technology.Methods From May 2020 to May 2021,the research subjects conveniently selected 80 primiparas who needed vaginal midwifery during vaginal delivery in the hospital,and were randomly divided into groups according to the digital table method.Among them,40 were the control group and 40 were the research group.The control group used episiotomy forceps to assist in delivery,while the study group used non-invasive forceps to assist in delivery.The maternal outcomes,neonatal outcomes,intrapartum hemorrhage,postpartum perineal pain,and pelvic floor function at 42 d postpartum were observed and recorded.And compared the observations of two groups.Results The incidence of maternal complications in the study group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).The incidence of neonatal complications between the study group and the control group was not statistically significant difference(P>0.05).The intrapartum hemorrhage volume of the study group was(202.27±11.62)mL,and the postpartum perineal pain score was(2.35±0.72)points,which was significantly less than that of the control group(302.83±13.01)mL,(3.81±0.76)points,the difference was statistically significant(t=36.460,8.820,P<0.05).The values of vaginal dynamic pressure,class I pelvic floor muscle strength,classⅡpelvic floor muscle strength,class I muscle fiber fatigue,and classⅡmuscle fiber fatigue were(85.32±3.56)cmH_(2)O and(3.46±0.42)grades(3.42±0.44)grade,(-0.04±0.01)%,(0.03±0.01)%,in the study group at 42 d postpartum,compared with the control group(81.18±3.71)cmH_(2)O,(2.52±0.41)grade,(2.54±0.43)grade,(-0.06±0.01)%,(0.05±0.02)%,the difference was statistically significant(t=5.092,10.129,9.046,8.944,5.657,P<0.05).Conclusion The clinical value of using non-invasive forceps in vaginal midwifery technology is very high,it can help improve maternal outcomes and pelvic floor function,and can also reduce intrapartum hemorrhage and postpartum perineal pain,which is worthy of clinical recommendation and application.
