中医熏洗联合针刺痔疮穴对缓解肛肠术后患者疼痛、水肿的临床效果研究

Study on the Clinical Effect of Fumigation and Washing of Traditional Chinese Medicine Combined with Acupuncture at Hemorrhoid Points on Relieving Pain and Edema in Patients after Anorectal Surgery

目的分析中医熏洗联合针刺痔疮穴在缓解肛肠术后患者疼痛、水肿中的应用效果。方法便利选择2019年2月-2021年7月期间该院收治的肛肠手术患者92例,随机分为对照组(n=46,高锰酸钾坐浴治疗)和观察组(n=46,中医熏洗联合针刺痔疮穴治疗)。对比两组术后疼痛程度、水肿程度、症状改善时间、住院时间、不良情绪评分、并发症和生活质量。结果两组术后1 d时的疼痛评分、水肿评分,差异无统计学意义(P>0.05);但观察组术后7、14 d时的疼痛评分、水肿评分,均低于对照组,差异有统计学意义(P<0.05);观察组患者术后症状改善时间和住院时间均短于对照组,差异有统计学意义(P<0.05);治疗后,观察组SDS评分(24.63±4.35)分、SAS评分(22.52±4.52)分均低于对照组(38.82±5.73)分、(34.87±5.26)分,差异有统计学意义(t=13.378、12.078,P<0.05);观察组患者术后的出血、感染、尿潴留、便秘等发生率显著低于对照组,差异有统计学意义(χ^(2)=5.845,P<0.05);观察组治疗后的各项生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论中医熏洗与针刺痔疮穴联合应用,可强化肛肠手术患者疼痛、水肿症状缓解效果,减少术后并发症,缩短其症状缓解时间和康复时间,从而有效减轻患者的不良情绪,提升患者的生活质量。

Objective To analyze the application effect of traditional Chinese medicine fumigation and washing combined with acupuncture on hemorrhoids points in relieving pain and edema of patients after anorectal surgery.Methods A total of 92 patients undergoing anorectal surgery in the hospital from February 2019 to July 2021 were conveniently selected.They were randomly divided into a control group(n=46,potassium permanganate bath therapy)and an observation group(n=46,Chinese medicine fumigation and washing combined with acupuncture treatment of hemorrhoids).The postoperative pain degree,edema degree,symptom improvement time,hospital stay,bad mood score,complications and quality of life were compared between the two groups.Results There was no significant difference in pain score and edema score 1 d after operation between the two groups(P>0.05).But the pain score and edema score at 7 d and 14 d after operation in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The improvement time of postoperative symptoms and hospitalization time in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the SDS score(24.63±4.35)points and SAS score(22.52±4.52)points in the observation group were lower than those in the control group(38.82±5.73)points and(34.87±5.26)points,the difference was statistically significant(t=13.378,12.078,P<0.05);the incidence of bleeding,infection,urinary retention and constipation in the observation group was significantly lower than that in the control group,and the difference was statistically significant(χ^(2)=5.845,P<0.05).After treatment,the quality of life scores in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The combined application of traditional Chinese medicine fumigation and acupuncture at hemorrhoid points can enhance the relief of pain and edema in patients undergoing anorectal surgery,reduce postoperative complications,and shorten the time of symptom relief and recovery.So as to effectively reduce the patient's bad mood and improve the patient's quality of life.
