快速康复外科理念联合Buerger运动在自体大隐静脉股-腘动脉转流术中的应用

Research of application of fast track surgery combined buerger exercise in autologous great saphenous femoral-popliteal artery bypass surgery

目的:探讨快速康复外科理念联合Buerger运动干预自体大隐静脉股-腘动脉转流术后的临床效果。方法:选取2015年1月—2020年1月于滨州市第二人民医院和山东第一医科大学附属省立医院血管外科接受自体大隐静脉股-腘动脉转流术的下肢动脉硬化闭塞症患者70例,随机分为观察组(n=35)和对照组(n=35),观察组采用快速康复外科理念联合Buerger运动的护理干预,对照组采用常规护理干预,对比两组术后生活质量、自理能力、踝肱压力指数、足背温度等。结果:手术后,两组患者生活质量评分、生活自理能力评分、踝肱压力指数、足背温度较治疗前均有显著改善,差异有统计学意义(P<0.05)。观察组术后生活质量评分、生活自理能力评分、足背温度以及术后3个月和6个月的踝肱压力指数优于对照组,差异有统计学意义(P<0.05)。结论:快速康复外科理念联合Buerger运动可以显著改善接受自体大隐静脉股-腘动脉转流术患者术后的生活质量,提高患者术后的自理能力,并能维持术后治疗效果。

Objective:To investigate the clinical effect on the intervention of fast track surgery combined Buerger exercise after autologous great saphenous femoral-popliteal artery bypass surgery.Method:Seventy patients with atherosclerosis obliterans who were treated in the department of vascular surgery in provincial hospital affiliated to Shandong First Medical University were separated into observation group(35 patients)and control group(35 patients)randomly.The fast track surgery combined Buerger exercise was performed in the observation group while the usual care was performed in the control group.The score of quality of life and activities of daily living,ankle-brachial index,the dorsalis pedis temperature were compared between 2 groups.Result:After surgery,the score of postoperative quality of life,activities of daily living,ankle brachial pressure index and the dorsalis pedis temperature of the two groups were significantly improved than pre-treatment,the difference was statistically significant(P<0.05).The post-operative score of postoperative quality of life,activities of daily living,the dorsalis pedis temperature and the 3 and 6 months post-operative ankle brachial pressure index of the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion:Fast track surgery combined Buerger exercise could improve the quality of life and activities of daily living,ankle brachial pressure index and the maximum blood flow velocity of bypass vein of patients underwent autologous great saphenous femoral-popliteal artery bypass surgery.
