医疗失效模式与效应分析在术中获得性压力性损伤风险管理中的应用

Application of Healthcare Failure Mode and Effect Analysis in the risk management of Intraoperation-Acquired Pressure Injury

目的:分析医疗失效模式与效应分析(HFMEA)在术中获得性压力性损伤(IAPI)风险管理中的应用效果。方法:组建IAPI项目HFMEA小组,绘制工作流程图,找出失效模式,分析潜在原因,制定行动方案并组织实施,最后进行效果评价。结果:应用HFMEA进行改进后,IAPI风险评估准确率、防压贴使用规范率、体位摆放规范率明显提高(P<0.05),IAPI发生率明显降低(P<0.05)。结论:HFMEA应用于IAPI的风险管理有利于规范针对IAPI发生的预防工作,可有效降低IAPI的发生率,保障患者安全。

Objective:To analyze the application of Healthcare Failure Mode and Effect Analysis(HFMEA)in the risk management of Intraoperation-Acquired Pressure Injury(IAPI).Methods:Established a HFMEA work team of IAPI project,made work flow chart,found out the failure mode in the risk management of IAPI,analyzed the potential root reasons,developed action plan and implementation,and evaluated the effect.Results:After applying HFMEA,the accuracy of IAPI risk assessment,the standard rate of using pressure-lowering pads and the standard rate of body position placement were significantly improved(P<0.05),and the incidence of IAPI was significantly decreased(P<0.05).Conclusion:The application of HFMEA in the risk management of IAPI can effectively help standardize the management process of IAPI prevention and decrease the incidence of IAPI,ensure patient safety.
