吴茱萸热熨敷配合穴位按摩对机械通气腹胀患者的疗效探讨

Evodia Hot Pressing Combined with Acupoint Massage on the Curative Effect of Mechanically Ventilated Patients with Abdominal Distension

目的探讨吴茱萸热熨敷配合穴位按摩对机械通气腹胀患者的影响。方法方便选取2019年6月—2020年12月接受治疗的76例机械通气并发腹胀患者,使用随机数字表法分为观察组38例和对照组38例。对照组予以常规护理及穴位按摩,观察组在对照组基础上结合吴茱萸热熨敷脐部。比较两组患者干预前后的肠鸣音次数、腹胀疗效评价、胃肠功能中医症状评分。结果干预3、6 d后,两组患者肠鸣音次数均明显改善,与干预前比较,差异有统计学意义(P<0.05),经重复方差测量分析表明,两组肠鸣音次数的时间效应、处理效应以及交互效应比较,差异有统计学意义(F=712.424、19.138、23.062,P<0.001);干预后观察组腹胀缓解有效率(88.89%)显著高于对照组(68.57%),干预6 d后两组患者胃肠功能中医症状评分均明显降低,与干预前比较差异有统计学意义(P<0.05),且观察组显著低于对照组,两组比较差异有统计学意义(t=-6.285,P<0.05)。结论吴茱萸热熨敷配合穴位按摩的干预有助于缓解机械通气腹胀患者的症状,提高肠鸣音次数,对于促进患者胃肠功能的恢复有积极作用。

Objective To investigate the effect of hot pressing of Evodia japonicus combined with acupoint massage on patients with abdominal distension in mechanical ventilation.Methods A total of 76 mechanically ventilated patients with abdominal distension who received treatment from June 2019 to December 2020 were conveniently selected and divided into an observation group of 38 cases and a control group of 38 cases using a random number table method.The control group received routine nursing and acupoint massage,and the observation group combined with Evodia hot pressing on the umbilicus on the basis of the control group.The number of bowel sounds before and after intervention,the evaluation of the efficacy of abdominal distension,and the TCM symptom score of gastrointestinal function were compared between the two groups of patients.Results After 3 d and 6 d after intervention,the number of bowel sounds in the two groups was significantly improved,and the difference was statistically significant(P<0.05),repeated measures analysis of variance showed that the time effect,treatment effect and interaction effect of the number of bowel sounds between the two groups were compared,and the difference was statistically significant(F=712.424,19.138,23.062,P<0.001);after the intervention,the effective rate of abdominal distension relief in the observation group(88.89%)was significantly higher than that in the control group(68.57%),after 6 d the intervention,the TCM symptom scores of gastrointestinal function of the two groups were significantly decreased,and the difference was statistically significant(P<0.05),and the observation group was significantly lower than the control group,and the difference between the two groups was statistically significant(t=-6.285,P<0.05).Conclusion The intervention of evodia hot pressing and acupoint massage can help relieve the symptoms of patients with abdominal distension in mechanical ventilation,increase the frequency of bowel sounds,and have a positive effect on promoting the recovery of gastrointestinal function in patients.
