摘要
目的分析血流限制训练联合重复经颅刺激对脑卒中后下肢功能的疗效。方法随机选取该院2020年7月—2021年7月诊治的60例脑卒中患者为研究对象,按数字均分法分为对照组与观察组,各30例。对照组患者在常规康复治疗的基础上行重复经颅刺激,观察组患者常规康复治疗的基础上行血流限制训练联合重复经颅刺激。比较两组患者治疗前后改良Barthel指数(MBI)、Fug-Meyer功能评定量表(LE-FMA)评分、下肢肌力改善状况、6 min步行距离测试(6 MWT)及治疗效果。结果治疗前,两组患者的MBI及LE-FMA评分对比,差异无统计学意义(t=0.458、0.327,P>0.05);治疗后,两组患者的MBI及LE-FMA评分均升高,且观察组评分均高于对照组,差异有统计学意义(t=5.119、5.441,P<0.05)。治疗后,观察组患者的下肢肌力改善状况高于对照组,差异有统计学意义(P<0.05)。观察组6 MWT高于对照组,差异有统计学意义(t=7.679,P<0.05)。观察组的治疗总有效率(96.67%)高于对照组(73.33%),差异有统计学意义(χ^(2)=4.706,P<0.05)。结论血流限制训练联合重复经颅刺激可提高脑卒中患者的日常生活活动能力、下肢运动能力,改善患者的下肢肌力,疗效显著,利于预后。
Objective To analyze the effect of blood flow restriction training combined with repeated transcranial stimulation on lower limb function after stroke.Methods 60 stroke patients diagnosed and treated in the hospital from July 2020 to July 2021 were randomly selected as the study objects.The patients were divided into control group and observation group,with 30 cases in each group.The patients in the control group repeated transcranial stimulation on the basis of routine rehabilitation treatment.The patients in the observation group received routine rehabilitation treatment based on upward blood flow restriction training combined with repeated transcranial stimulation.MBI,le-fma score,improvement of lower limb muscle strength,6-minute walking distance test(6MWT)and treatment effect were compared between the two groups before and after treatment.Results The MBI and le-fma scores of the two groups before treatment,the difference was not statistically significant(t=0.458,0.327,P>0.05).After treatment,the scores of MBI and le-fma in the two groups increased,and the scores in the observation group were higher than those in the control group,the difference was statistically significant(t=5.119,5.441,P<0.05).After treatment,the improvement of lower limb muscle strength in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).6MWT in the observation group was higher than that in the control group,the difference was statistically significant(t=7.679,P<0.05).The total effective rate of the observation group(96.67%)was higher than that of the control group(73.33%),the difference was statistically significant(χ^(2)=4.706,P<0.05).Conclusion Blood flow restriction training combined with repeated transcranial stimulation can improve the activities of daily living,lower limb motor ability and lower limb muscle strength of stroke patients.The curative effect is significant and conducive to the prognosis.
