通络解痉汤联合针灸治疗脑卒中痉挛性偏瘫患者的临床效果

Clinical Effect of Tongluo Jiejing Decoction Combined with Acupuncture on Stroke Patients with Spastic Hemiplegia

目的:探讨通络解痉汤联合针灸治疗脑卒中痉挛性偏瘫患者的临床效果。方法:将86例脑卒中痉挛性偏瘫患者遵循随机数字表法的分组原则均分为两个组别:治疗组(n=43)和对照组(n=43)。所有患者均采用常规的西医治疗措施,对照组加用针灸治疗,治疗组采用针灸与通络扶正汤联合治疗的措施。对临床疗效进行比较,评价改良Barthel指数、Fugl-Meyer平衡功能、膝关节肌张力(MAS)评分治疗前后的变化。结果:治疗组总有效人数40例,总有效率为93.02%;对照组总有效人数32例,总有效率为74.42%;治疗组的较对照组显著提高(P<0.05)。Barthel指数及Fugl-Meyer评分经过治疗后,两组患者的均升高(P<0.05)。治疗组患者治疗后的Barthel指数及Fugl-Meyer评分高于对照组(P<0.05)。患肢肘关节及膝关节MAS评分经过治疗后,两组患者的均降低(P<0.05),治疗组患者治疗后肘关节及膝关节MAS评分较对照组更低(P<0.05)。结论:脑卒中痉挛性偏瘫患者在常规西医治疗的同时加用通络解痉汤与针灸治疗,患肢痉挛程度、关节肌肉张力及日常生活能力均提高,治疗效果令人满意。

Objective:To explore the clinical effect of Tongluo Jiejing decoction combined with acupuncture on stroke patients with spastic hemiplegia.Method:86 stroke patients with spastic hemiplegia were divided into two groups according to the grouping principle of random number table:treatment group(n=43)and control group(n=43).All patients were treated with conventional western medicine,the control group was treated with acupuncture,and the treatment group was treated with acupuncture and Tongluo Fuzheng decoction.The clinical effects were compared,and the changes of modified Barthel index,Fugl Meyer balance function and knee muscle tension(MAS)score before and after treatment were evaluated.Result:The total number of effective patients in the treatment group was 40,and the total effective rate was 93.02%;In the control group,the total effective number was 32 cases,and the total effective rate was 74.42%;The of the treatment group was significantly higher than that of the control group(P<0.05).Barthel Index and Fugl Meyer score increased after treatment(P<0.05).The Barthel Index and Fugl Meyer score in the treatment group were higher than those in the control group(P<0.05).After treatment,the MAS scores of elbow and knee joints in the two groups decreased(P<0.05).The MAS scores of elbow and knee joints in the treatment group were lower than those in the control group(P<0.05).Conclusion:Stroke patients with spastic hemiplegia were treated with Tongluo Jiejing Decoction and acupuncture while routine western medicine.The degree of spasticity,joint muscle tension and daily living ability of the affected limbs were improved,and the treatment effect was satisfactory.
