颈动脉多普勒超声评估心血管疾病高发群体颅内动脉狭窄性病变的临床效果 被引量:3

摘要 目的:探讨颈动脉多普勒超声评估对高发群体颅内动脉狭窄性病变的临床价值。方法:本次试验选取我院2019年12月—2021年10月收治的心血管疾病高发患者62例进行研究,给予所有患者颈动脉多普勒超声及数字减影血管造影(DSA)检查,所选用的“金标准”为后者检查结果,分析颅内动脉狭窄性病变诊断中颈动脉多普勒超声评估的临床价值。结果:经DSA检查,结果显示62例疑似心血管疾病高发群体颅内动脉狭窄性病变患者中阳性38例,占比61.29%,阴性24例,占比38.71%;多普勒超声检查准确度为93.55%,阴性预测值为92.31%,明显高于颈动脉超声检查80.65%的准确度及71.43%的阴性预测值,组间差异显著(P<0.05)。结论:颈动脉多普勒超声相比DSA检查技术在评估心血管疾病高发群体颅内动脉狭窄性病变方面,准确度与灵敏度一致性较高,实践价值良好。 Objective:To evaluate the clinical value of carotid doppler ultrasonography in patients with high incidence of intracranial arterial stenosis.Methods:62 patients with high incidence of cardiovascular disease admitted to our hospital from December 2019 to October 2021 were selected for the study.All patients were examined by carotid Doppler ultrasound and digital subtraction angiography(DSA).The“gold standard”was the result of the latter examination.To analyze the clinical value of carotid doppler ultrasonography in diagnosis of intracranial arterial stenosis.Results:DSA examination showed that 38 cases(61.29%)were positive and 24 cases(38.71%)were negative in 62 patients with intracranial arterial stenosis in suspected high risk group of cardiovascular disease.The accuracy of Doppler ultrasonography was 93.55%and the negative predictive value was 92.31%,which were significantly higher than that of carotid ultrasonography(80.65%and 71.43%),and the difference between groups was significant(P<0.05).Conclusions:Compared with DSA,carotid Doppler ultrasound has a higher consistency of accuracy and sensitivity in the assessment of intracranial arterial stenosis in patients with high risk of cardiovascular disease,and has good practical value.
