高频彩色多普勒超声在小儿先天性肌性斜颈诊断及治疗后效果的价值

Value of High-Frequency Color Doppler Ultrasonography in the Diagnosis and Treatment of Children with Congenital Muscular Torticollis

目的:探究高频彩色多普勒超声(以下简称“高频彩超”)在小儿先天性肌性斜颈(CMT)诊断及治疗后效果的应用价值。方法:选择2017年8月—2020年8月我院收治的疑似CMT患儿60例作为研究对象。所有患儿均行高频彩超检查及推拿治疗。分析高频彩超检查结果及超声表现,以及不同时期CMT超声表现及治疗效果。结果:60例患儿中,经检查56例患儿出现异常,另4例患儿无胸锁乳突肌异常表现。病变位于右侧26例,左侧30例;经高频彩超检查可见肿块型先天性斜颈患儿26例,弥漫型先天性肌性斜颈15例,混合型先天性肌性斜颈15例;依据患儿年龄分为早期组(年龄<1岁) 45例,后期组(年龄≥1岁) 11例;患侧组早期及后期胸锁乳突肌回声不均、血流稀疏发生率高于健侧对照组,横径大于健侧对照组,差异有统计学意义(P<0.05);56例患儿经推拿治疗后治愈32例,显效17例,无效7例,治疗总有效率为87.50%。结论:高频彩超在CMT诊断中应用效果确切,能够观察病变特征变化,给予针对性治疗后获得较好的治疗效果,值得推广。

Objective:To explore the application value of high-frequency color Doppler ultrasound(hereinafter referred to as“high-frequency color Doppler”)in the diagnosis and treatment of children with congenital muscular torticollis(CMT).Methods:A total of 60 children with suspected CMT admitted to our hospital from August 2017 to August 2020 were selected as the research subjects.All patients underwent high-frequency color Doppler ultrasound examination and massage therapy.The results of high-frequency color Doppler ultrasonography and ultrasonography,as well as CMT ultrasonography in different periods and treatment effects were analyzed.Results:Among the 60 children,56 children showed abnormality,and the other 4 children had no abnormality of sternocleidomastoid muscle.The lesions were located in 26 cases on the right side and 30 cases on the left side;26 cases of children with mass congenital torticollis,15 cases of diffuse congenital muscular torticollis,and 15 cases of mixed congenital muscular torticollis were found by high-frequency color Doppler ultrasonography;According to the age of the children,they were divided into an early group(age<1 year),45 cases,and a late group(age≥1 year),11 cases;the sternocleidomastoid muscle had a high incidence of uneven echo and sparse blood flow in the early and late stages of the ipsilateral group.In the healthy side control group,the transverse diameter was larger than that of the healthy side control group,and the difference was statistically significant(P<0.05);32 cases of 56 children were cured after massage treatment,17 cases were markedly effective,and 7 cases were ineffective.The total effective rate was 87.50%.Conclusion:The application effect of high-frequency color Doppler ultrasound in the diagnosis of CMT is accurate,it can observe the changes of lesion characteristics,and obtain better therapeutic effect after targeted treatment,which is worthy of promotion.
