乳腺癌改良根治术中保留肋间臂神经对乳腺癌患者术后并发症及功能恢复的影响

The Effect of Preservation of Intercostal Brachial Nerve in Modified Radical Mastectomy for Breast Cancer on Postoperative Complications and Function Recovery of Breast Cancer Patients

目的:探讨乳腺癌改良根治术(MRM-BC)中保留肋间臂神经(ICBN)在乳腺癌患者中的效果。方法:选取某院2018年7月-2021年7月诊治的100例女性乳腺癌患者,按随机数字表法分为对照组和观察组各50例,所有患者均行MRM-BC,对照组术中切除ICBN,观察组术中保留ICBN,观察至术后3个月。对比两组上臂内侧感觉障碍发生情况、并发症、生活质量。结果:两组并发症发生率相比,无统计学差异(P>0.05);术后1、3个月,观察组上臂内侧感觉障碍发生率较对照组低,有统计学差异(P<0.05);术后,观察组生活质量综合评定问卷(GQOLI-74)GQOLI-74内各维度评分较对照组高,有统计学差异(P<0.05)。结论:MRM-BC中保留ICBN不会增加并发症发生风险,并能够有效减少上臂内侧感觉障碍的发生,有利于感觉功能的恢复,进而提高患者生活质量,安全可靠,临床可大力推行。

Objective:To investigate the effect of preservation of the intercostal brachial nerve(ICBN)in modified radical mastectomy for breast cancer(MRM-BC)in breast cancer patients.Methods:A total of 100 female breast cancer patients diagnosed and treated in a hospital from July 2018 to July 2021 were selected and divided into two groups according to the random number table method,with 50 cases in each group.All patients underwent MRM-BC.The control group underwent intraoperative resection of ICBN,and the observation group retained ICBN during the operation.The two groups were observed until 3 months after surgery.The incidence of the upper arm medial sensory disturbance,complications and quality of life were compared between the two groups.Results:There was no significant difference in the incidence of complications between the two groups(P>0.05).1 and 3 months after operation,the incidence of upper arm medial sensory disturbance in the observation group was lower than that in the control group,with statistical difference(P<0.05).After operation,the scores of all dimensions in the GQOLI-74 of the Comprehensive Assessment Questionnaire for Quality of Life(GQOLI-74)in the observation group were higher than those in the control group,with statistical difference(P<0.05).Conclusion:Retaining ICBN in MRM-BC will not increase the risk of complications,and can effectively reduce the occurrence of sensory disturbances in the medial upper arm,which is conducive to the recovery of sensory function,thereby improving the quality of life of patients,with safety and reliability,and can be vigorously promoted in clinical practice.
