微生态制剂结合抗Hp三联方案治疗Hp阳性腹型AP患儿的效果 被引量:1

Effect of Probiotics Combined with Anti Hp Triple Regimen on Children with Hp-positive Abdominal AP

摘要 目的:探讨微生态制剂结合抗幽门螺杆菌(Hp)三联方案治疗Hp阳性腹型过敏性紫癜(Ap)患儿的效果。方法:将某院2020年1月-2021年10月期间收治的61例Hp阳性腹型AP患儿按照随机数字表法分为常规组30例和联合组31例,常规组采用常规抗Hp三联方案治疗,联合组在常规组基础上采用微生态制剂治疗,观察两组患儿临床疗效、肠道菌群数、炎症因子水平以及不良反应发生率。结果:治疗2周后,联合组总有效率(93.55%)高于常规组(70.00%)(P<0.05);联合组双歧杆菌(7.63±0.83)/g、乳酸杆菌(6.47±0.74)/g、粪球菌(5.67±0.94)/g水平高于常规组(6.98±0.86)/g、(5.93±0.73)/g、(4.96±0.88)/g,联合组大肠杆菌(5.92±0.69)/g水平低于常规组(6.37±0.74)/g(P<0.05);联合组PCT(2.28±0.59)μg/L、TNF-α(4.82±1.15)μg/L、IL-4(14.67±4.08)pg/mL水平低于常规组(2.79±0.62)μg/L、(5.95±1.36)μg/L、(18.25±5.11)pg/mL(P<0.05);联合组不良反应发生率与常规组对比差异无统计学意义(9.68%vs 13.33%)(P>0.05)。结论:对Hp阳性腹型Ap患儿,联合应用微生态制剂治疗有助于改善肠道菌群,缓解肠道炎症反应,提高临床疗效,且不增加不良反应发生率。 Objective:To investigate the effect of probiotics combined with anti Helicobacter pylori(Hp)triple regimen on children with Hp-positive abdominal allergic purpura(AP).Methods:61 children with Hp-positive abdominal AP treated in a hospital from January 2020 to October 2021 were randomly divided into combined group and conventional group.30 cases in the conventional group were treated with conventional anti Hp triple regimen,and 31 cases in the combined group were treated with probiotics on the basis of the conventional group.The clinical efficacy,intestinal flora,inflammatory factors levels and incidence of adverse reactions of the two groups were observed.Results:After 2 weeks of treatment,the total effective rate of the combined group(93.55%)was higher than that of the conventional group(70.00%)(P<0.05).The levels of Bifidobacterium[(7.63±0.83)/g],Lactobacillus[(6.47±0.74)/g]and Enterococcus faecalis[(5.67±0.94)/g]in the combined group were higher than those in the conventional group[(6.98±0.86)/g,(5.93±0.73)/g and(4.96±0.88)/g],and the level of Escherichia coli[(5.92±0.69)/g]in the combined group was lower than that in the conventional group[(6.37±0.74)/g](P<0.05).The levels of PCT[(2.28±0.59)μg/L],TNF-α[(4.82±1.15)μg/L]and IL-4[(14.67±4.08)pg/mL]in the conventional group were lower than those in the routine group[(2.79±0.62)μg/L,(5.95±1.36)μg/L,(18.25±5.11)pg/mL](P<0.05).There was no significant difference in the incidence of adverse reactions between the combined group and the conventional group(9.68%vs 13.33%)(P>0.05).Conclusion:For children with Hp-positive abdominal AP,the combined application of probiotics is helpful to improve intestinal flora,alleviate intestinal inflammatory reaction,and improve clinical efficacy,without increasing the incidence of adverse reactions.
