摘要
目的:研究多途径脑血流动力学改善的实施方法及在康复期脑梗塞患者护理中的应用价值。方法:将某院收治的120例康复期脑梗塞患者随机分为C组和C+N组各60例,C组采用常规护理,C+N组在C组的基础上采用多途径脑血流动力学改善干预。于护理前后分别检测脑血流动力学指标(基底动脉血流速度、左/右椎动脉血流速度)、血清神经功能指标[神经特异性蛋白(S100β)、脂蛋白相关磷脂酶(Lp-PLA2)、神经元特异性烯醇化酶(NSE)],并根据GOS(格拉斯哥预后评分)评分评价患者临床预后。结果:护理后,C+N组患者基底动脉血流速度、左/右椎动脉血流速度均大于C组(P<0.05),C+N组患者血清S100β、Lp-PLA2、NSE均低于C组(P<0.05);C+N组患者临床预后良好率为70.00%,高于C组的56.67%(P<0.05)。结论:予以康复期脑梗塞患者多途径脑血流动力学改善护理可有效改善患者脑组织血供,减轻神经功能损伤程度,改善临床预后。
Objective:To study the implementation method of multi-pathway cerebral hemodynamics improvement and its application value in the nursing of patients with cerebral infarction in rehabilitation period.Methods:120 patients with convalescent cerebral infarction admitted to a hospital were randomly divided into group C(60 cases)and group C+N(60 cases).Group C was given routine nursing,and group C+N was given multiway cerebral hemodynamics improvement intervention on the basis of group C.Cerebral hemodynamic indexes(basilar artery blood flow velocity,left and right vertebral artery blood flow velocity),and serum neurological function indexes[neurospecific protein(S100β),lipoprotein-associated phospholipase(LP-PLA2),neuron-specific enolase(NSE)]were detected before and after nursing.The clinical prognosis was evaluated according to GOS(Glasgow outcome Scale)score.Results:After nursing,the blood flow velocity of basilar artery and left/right vertebral artery in C+N group were higher than those in C group(P<0.05),and the serum S100β,LP-PLA2 and NSE in C+N group were lower than those in C group(P<0.05).The good clinical prognosis rate in C+N group was 70.00%,which was higher than 56.67%in C group(P<0.05).Conclusion:Multiway cerebral hemodynamics improvement nursing for patients with cerebral infarction in rehabilitation period can effectively improve the blood supply of brain tissue,reduce the degree of nerve function injury and improve the clinical prognosis.
