凤香洗液在子宫全切术后阴道残端愈合中的疗效分析

Curative Effect of Fengxiang Lotion on Vaginal Stump Healing after Total Hysterectomy

目的:观察腹腔镜下全子宫切除术后阴道残端愈合情况及中药凤香洗液局部应用的干预作用。方法:回顾性分析2019年1月-2021年6月在陕西中医药大学附属医院行全子宫切除术患者的资料,共选取282例,分为观察组和对照组,观察组141例,于术后第3~7天给予中药凤香洗液局部冲洗阴道;对照组141例,于术后第3~7天给予甲硝唑氯化钠注射液冲洗阴道,每组用药每天1次,连续用药5d,术后30d比较两组术后阴道残端愈合时阴道出血率、息肉形成情况及阴道菌群失调率。结果:术后30d,观察组阴道分泌物清洁度(Ⅲ~Ⅳ)、细菌性阴道病、霉菌性阴道炎检出率及pH阳性率均低于对照组,能够改善阴道内环境,两组比较差异有统计学意义(P<0.05);观察组阴道残端愈合程度高于对照组,阴道息肉形成率低于对照组,两组比较差异有统计学意义(P<0.05)。结论:阴道局部使用凤香洗液可有效预防子宫全切术后阴道残端的出血、息肉形成及菌群失调,促进阴道残端的愈合,防止阴道炎的发生,适合临床推广应用。

Objective:To observe the healing of vaginal stump after total hysterectomy under laparoscope and the intervention effect of local application of Fengxiang Lotion.Methods:282 patients with total hysterectomy were randomly divided into the observation group and the control group,with 141 patients in each group.The observation group was given local vaginal irrigation with Fengxiang Lotion,whereas the control group was given Metronidazole Sodium Chloride Injection for vaginal irrigation on 3~7 days after the operation.The intervention was once a day for 5 consecutive days in each group.The vaginal bleeding rate during postoperative vaginal stump healing,polyp formation and vaginal microbiome imbalance rate were compared between the two groups on the 30th day after the operation.Results:30 days after the operation,the cleanliness of vaginal secretions(Ⅲ-Ⅳ),the detection rate of bacterial vaginosis and mycotic vaginitis,as well as pH positive rate were lower in the observation group than those in the control group(P<0.05).The healing degree of vaginal stump was higher in the observation group than that in the control group,and the rate of vaginal polyp formation was lower in the observation group than that in the control group(P<0.05).Conclusion:Local application of Fengxiang Lotion can effectively prevent vaginal stump bleeding,polyp formation and bacterial disorders after total hysterectomy,it can promote the healing of vaginal stump and prevent the occurrence of vaginitis,and it’s worthy of clinical application.
