2009—2021年天津市西青区医疗机构消毒效果监测与影响因素分析

Monitoring and influencing factors of disinfection effect in medical institutions in Xiqing District,Tianjin from 2009 to 2021

目的了解天津市西青区各级各类医疗机构消毒效果,分析消毒效果监测结果变化趋势及其影响因素。方法2009—2021年连续13年对该区医疗机构进行消毒效果监测,以χ^(2)趋势检验分析合格率的年度变化趋势,并对各级各类医疗机构监测结果进行横向对比。结果2009—2021年,共采集样本10946件,合格10747件,总合格率为98.18%。不同年度间合格率差异有统计学意义,全区总体合格率呈上升趋势(Z=4.64,P=0.031)。其中,三级医疗机构物体表面合格率最高(99.81%);民营医疗机构中,医务人员手(92.71%)和紫外线灯强度(95.24%)合格率均较低。三级医疗机构Ⅱ类环境空气合格率最高(98.04%);一级医疗机构Ⅲ、Ⅳ类环境空气合格率最高(99.05%)。结论天津西青区各级医疗机构消毒效果总体合格率处于较高水平,应进一步加强院内消毒知识培训,保障感控和监测队伍人员稳定,形成医疗机构内消毒与感染防控的长效机制。

Objective To understand the disinfection effect of various medical institutions at all levels in Xiqing District,Tianjin and to analyze the change trend and influencing factors of the monitoring results of disinfection effect.Methods From 2009 to 2021,the disinfection effect of medical institutions in the district was monitored for 13 consecutive years.The annual trend of the qualified rate was analyzed by chi-square trend test,and the monitoring results of various medical institutions at all levels were compared horizontally.Results From 2009 to 2021,a total of 10946 samples were collected,of which 10747 were qualified,with a total qualified rate of 98.18%.The differences of qualified rates among different years were statistically significant,and the overall qualified rate of the whole district showed an upward trend(Z=4.64,P=0.031).Among them,the qualified rate of object surfaces in tertiary medical institutions was the highest(99.81%);in private medical institutions,the qualified rates of medical staff’s hands(92.71%)and ultraviolet lamp intensity(95.24%)were the lowest.The qualified rate of ClassⅡambient air in tertiary medical institutions was the highest(98.04%);the qualified rate of ClassⅢandⅣambient air in first-level medical institutions was the highest(99.05%).Conclusion The overall qualified rate of disinfection effect of medical institutions at all levels in Xiqing District,Tianjin is at a high level.The training of in-hospital disinfection knowledge should be further strengthened to ensure the stability of infection control and monitoring team personnel.A long-term mechanism for disinfection and infection prevention and control in medical institutions should be formed.
