脑出血患者谷氨酸、CRP及S-100β蛋白水平与神经功能恢复、预后的关系分析 被引量:1

RELATIONSHIPS BETWEEN GLUTAMATE,CRP AND S-100ΒPROTEIN LEVELS,NEUROLOGICAL RECOVERY AND PROGNOSIS IN PATIENTS WITH CEREBRAL HEMORRHAGE

摘要 目的分析脑出血患者谷氨酸(Glu)、C反应蛋白(CRP)及S-100β蛋白水平与神经功能恢复、预后的关系。方法收集2019年1月—2020年2月该院收治的脑出血患者81例,设为观察组,根据患者NIHSS评分分为轻度组26例、中度组31例、重度组24例。另选取本院同期行健康体检的志愿者78例作为健康对照组。观察不同人群以及患者入院后24 h、3 d、7 d后Glu、CRP及S-100β蛋白水平,比较不同神经功能损伤以及不同预后患者中Glu、CRP及S-100β蛋白水平,分析Glu、CRP及S-100β与ADL量表、NIHSS量表相关性。结果观察组Glu、CRP及S-100β蛋白水平均明显高于对照组(P<0.05);在不同时间段脑出血患者中Glu、CRP及S-100β蛋白表达水平为发病后3d>发病后7d>发病后24d(P<0.05),不同神经功能损伤患者Glu、CRP及S-100β蛋白水平均为重度组>中度组>轻度组(P<0.05);81例患者治疗后30d根据ADL评分分为预后良好35例,预后不良46例,预后不良组患者Glu、CRP及S-100β蛋白水平明显高于预后良好组(P<0.05);经Pearson相关性分析,Glu、CRP及S-100β与ADL量表呈负相关,与NIHSS量表呈正相关(P<0.05)。结论脑出血患者Glu、CRP及S-100β蛋白水平与其神经功能损伤程度密切相关,可为患者预后评估提供重要参考信息,就有较高的临床使用价值。 Objective To analyze the relationships between Glutamate(Glu),C-reactive protein(CRP)and S-100βprotein levels,neurological recovery and prognosis in patients with cerebral hemorrhage.Methods Eighty-one patients with cerebral hemorrhage admitted to Meishan People′s Hospital from January 2019 to February 2020 were collected and set as the observation group.According to the patients′NIHSS score,they weredivided into:26 cases in mild group,31 cases in moderate group,and 24 cases in severe group.In addition,78 volunteers who had physical examinations in the same period in this hospital were selected as the healthy control group.The levels of Glu,CRP and S-100βprotein 24 h,3 d and 7 d after admission to the hospital in different populations and parents were compared.The levels of Glu,CRP,and S-100βprotein in patients with different neurological impairments and different prognosis were compared,and the correlations between the levels of Glu,CRP,and S-100β,the ADL scale and NIHSS scale was analyzed.Results The levels of Glu,CRP and S-100βprotein in the observation group were significantly higher than those in the control group(P<0.05).The protein expression levels of Glu,CRP and S-100βin patients with cerebral hemorrhage at different time periods were 3 days after the onset of the disease>7 days after the onset of the disease>24 d after the onset(P<0.05),The levels of Glu,CRP and S-100βprotein in patients with different neurological impairments were as follows:severe grouP>moderate grouP>mild group(P<0.05).According to the ADL score 30 days after treatment,among 81 patients,35 had a good prognosis and 46 had a poor prognosis.The levels of Glu,CRP and S-100βprotein in the poor prognosis group were significantly higher than those in the good prognosis group(P<0.05).Pearson correlation analysis showed that Glu,CRP and S-100βwere negatively correlated with the ADL scale,and positively correlated with the NIHSS scale(P<0.05).Conclusion The levels of Glu,CRP and S-100βprotein in patients with cerebral hemorrhage are closely related to the degree of neurological damage,which can provide important reference information for the prognosis assessment of patients,and has high clinical value.
