冠脉CTA在冠心病临床诊断中的应用

Application of Coronary Artery CTA in Clinical Diagnosis of Coronary Heart Disease

目的:分析冠状动脉粥样硬化性心脏病(简称,冠心病)疾病特点,评价冠脉CT血管成像(CTA)诊断的价值,为冠心病临床诊断工作提供参考。方法:选择2019年5月~2020年3月本院诊治的80例冠心病患者,采取冠脉CTA诊断(观察1组)以及冠脉造影检查(观察2组)。比较经不同检查后冠心病患者检查结果、左冠脉主血管、左前降血管、左回旋血管、右冠脉检出情况以及冠脉CTA诊断冠状动脉狭窄程度(轻、中、重度狭窄以及闭塞)。结果:与冠脉造影检查的观察2组比较,冠脉CTA诊断的观察1组冠心病患者的左冠脉主血管(25.00%)、左前降血管(25.00%)、左回旋血管(20.00%)、右冠脉(20.00%)以及总检出率(90.00%)差异无显著性(P均>0.05)。另外,观察1组冠状动脉狭窄程度方面,中度、重度狭窄占比率较高,高于轻度狭窄、闭塞率。结论:冠心病诊断中冠脉CTA诊断以及冠脉造影诊断结果接近,对比冠脉造影诊断,冠脉CTA诊断方便且无创,能有效显示冠状动脉狭窄情况,为冠心病临床治疗提供参考。

Objective:To analyze the characteristics of coronary heart disease(CHD)and evaluate the diagnostic value of CT angiography(CTA),so as to provide reference for the clinical diagnosis of CHD.Methods:80 patients with CHD who were diagnosed and treated in our hospital from May 2019 to March 2020 were selected for diagnosis of coronary artery CTA(observation group 1)and coronary angiography(observation group 2).The examination results of patients with CHD after different examinations,the detection of the left main vessel,the left anterior descending vessel,the left circu lar vessel and the right coronary artery,and the degree of coronary artery stenosis(mild,moderate and severe stenosis and occlusion)diagnosed by coronary artery CTA were compared.Results:Compared with the observation group 2 by coronary angiography,the left coronary main vessel(25.00%),the left anterior descending vessel(25.00%)and the left circumflex vessel(20.00%)in the observation group 1 with coronary heart disease diagnosed by coronary CTA,right coronary artery(20.00%)and total detection rate(90.00%)had no significant difference(P>0.05).In addition,in terms of the degree of coronary artery stenosis in group 1,the proportion of moderate and severe stenosis was higher than that of mild stenosis and occlusion.Conclusion:In the diagnosis of CHD,the results of coronary CTA diagnosis and coronary angiography diagnosis are similar.Compared with the diagnosis of coronary angiography,the diagnosis of coronary CTA is convenient and noninvasive,and can effectively display the situation of coronary artery stenosis,which provides reference for the clinical treatment of CHD.
