富血小板血浆治疗早期股骨头缺血性坏死的研究进展

Progress of hip preservation therapy for early avascular necrosis of femoral head based on platelet-rich plasma

股骨头缺血性坏死(avascular necrosis of the femoral head,ANFH)主要表现为股骨头的缺血性病变、骨细胞坏死、微骨折和关节面塌陷。目前,早期ANFH主要的手术治疗方式是髓芯减压,然而其疗效仍有很大的改善空间。为了提高髓芯减压效率,再生技术如富血小板血浆(platelet-rich plasma,PRP)被建议用于早期ANFH。作为一种血小板浓度远高于基线的自体血浆,PRP在组织修复、再生和间充质干细胞分化中起重要作用,近年来对早期ANFH的治疗也成为研究热点。本文就PRP的基本特征、作用机制和临床应用的相关研究进行综述。

Avascular necrosis of the femoral head(ANFH)is characterized by ischemic lesions of the femoral head,osteocyte necrosis,microfractures,and articular surface collapse.At present,the main surgical treatment for early ANFH is core decompression,but there is still some room for improvement.In order to improve the efficiency of core decompression,regenerative techniques such as platelet-rich plasma(PRP)are suggested for early ANFH.As a kind of autologous plasma with platelet concentration much higher than the baseline,PRP plays an important role in tissue repair,regeneration and differentiation of mesenchymal stem cells,and its therapeutic effect on early ANFH has also become a research focus in recent years.This article reviews the basic features,mechanism of action and clinical application of PRP.
