儿童重症病毒性肺炎患儿幽门螺杆菌感染与T淋巴细胞亚群的相关性分析 被引量:1

摘要 目的通过检测儿童重症病毒性肺炎患儿幽门螺杆菌(H.pylori)、T淋巴细胞亚群,探讨H.pylori感染与T淋巴细胞亚群的相关性。方法选择2020年1月至2021年8月三门峡市中心医院儿童重症医学科收治的240例儿童重症病毒性肺炎患儿为研究对象,依据H.pylori检测结果将其分为对照组(H.pylori感染阴性)和观察组(H.pylori感染阳性),每组120例。比较两组的H.pylori、血红蛋白(Hb)、降钙素原(PCT)水平以及T淋巴细胞亚群(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、Th17、Treg、Th17/Treg),分析H.pylori感染与T淋巴细胞亚群、Hb、PCT的相关性。结果观察组的H.pylori、PCT水平高于对照组,Hb水平低于对照组,差异具有统计学意义(P<0.05);观察组的CD4^(+)、CD4^(+)/CD8^(+)、Treg均低于对照组,CD8^(+)、Th17、Th17/Treg高于对照组,差异具有统计学意义(P<0.05);Pearson相关性分析结果显示,H.pylori与CD4^(+)、CD4^(+)/CD8^(+)、Treg、Hb呈负相关(r=-0.134、-0.186、-0.163、-0.076,P<0.05),与CD8^(+)、Th17、Th17/Treg、PCT呈正相关(r=0.105、0.232、0.271、0.159,P<0.05)。结论儿童重症病毒性肺炎合并H.pylori感染时机体免疫功能显著下降,其机制与H.pylori感染密切相关。 Objective To explore the correlation between Helicobacter pylori(H.pylori)infection and T lymphocyte subsets by detecting H.pylori and T lymphocyte subsets in children with severe viral pneumonia.Methods A total of 240 children with severe viral pneumonia treated in the pediatric intensive care medicine department of Sanmenxia central hospital from January 2020 to August 2021 were selected as the research objects.According to the test results of H.pylori,the children were divided into control group(negative H.pylori infection)and observation group(positive H.pylori infection),with 120 cases in each group.The H.pylori,hemoglobin(Hb),procalcitonin(PCT)levels and T lymphocyte subsets(CD4^(+),CD8^(+),CD4^(+)/CD8^(+),Th17,Treg and Th17/Treg)were compared between the two groups,and the correlation between H.pylori infection and T lymphocyte subsets,Hb and PCT were analyzed.Results The H.pylori and PCT level in the observation group were higher than those in the control group,and the Hb level was lower than that in the control group,the differences were statistically significant(P<0.05);CD4^(+),CD4^(+)/CD8^(+)and Treg in the observation group were lower than those in the control group,while CD8^(+),Th17 and Th17/Treg were higher than those in the control group,the differences were statistically significant(P<0.05);Pearson correlation analysis results showed that H.pylori was negatively correlated with CD4^(+),CD4^(+)/CD8^(+),Treg and Hb(r=-0.134,-0.186,-0.163,-0.076,P<0.05),and positively correlated with CD8^(+),Th17,Th17/Treg and PCT(r=0.105,0.232,0.271,0.159,0.193,P<0.05).Conclusion The immune function of children with severe viral pneumonia complicated with H.pylori infection decreased significantly,and its mechanism is closely related to H.pylori infection.
