2型糖尿病肾病与非糖尿病肾病患者自体动静脉内瘘术后成熟度的差异

Differences in Postoperative Maturity of Autologous Arteriovenous Fistula in Patients with Type 2 Diabetic Nephropathy and Non-diabetic Nephropathy

目的探讨2型糖尿病肾病患者同非糖尿病肾病患者呈现出的自体动静脉内瘘术后成熟度差异。方法选取2018年1月—2020年12月收治的65例2型糖尿病肾病患者为研究组,同期收治的65例终末期肾脏病患者作为参照组。对两组患者血流动力学参数以及内瘘成熟状态差异性展开对比。结果研究组自体动静脉内瘘距皮距离及头静脉管壁厚度同参照组比较,差异无统计学意义(P>0.05);研究组血流量、头静脉直径及成熟时间分别少于、小于以及长于参照组,差异有统计学意义(P<0.05);研究组术后不同时间段头静脉血流量均低于参照组,差异有统计学意义(P<0.05);研究组术后不同时间段头静脉内径均小于参照组,差异有统计学意义(P<0.05)。结论同非糖尿病患者比较,糖尿病肾病患者血流动力学及自体动静脉内瘘术后成熟度表现出显著差异,对此针对自体动静脉内瘘成熟状态进行准确判断,对于透析治疗适时开展及维持血液透析顺利开展表现出显著价值。

Objective To investigate the difference of postoperative maturity of autologous arteriovenous fistula in patients with type 2 diabetic nephropathy and non-diabetic nephropathy.Methods A total of 65 patients with type 2 diabetic nephropathy who were admitted from January 2018 to December 2020 were selected as the study group.A total of 65 patients with end-stage renal disease who were treated during the same period served as the reference group.The differences in hemodynamic parameters and the maturity status of internal fistulas were compared between the two groups.Results Compared with the reference group,there was no significant difference in the distance from the skin of the autogenous arteriovenous fistula and the thickness of the cephalic vein wall in the study group,the difference was not statistically significant(P>0.05).The blood flow,cephalic vein diameter and maturation time in the study group were less,less and longer than those in the reference group,the difference was statistically significant(P<0.05).The cephalic venous blood flow in the study group was lower than that in the reference group at different time periods after operation,the difference was statistically significant(P<0.05).The inner diameter of the cephalic vein in the study group was smaller than that in the reference group at different time periods after operation,the difference was statistically significant(P<0.05).Conclusion Compared with non-diabetic patients,diabetic nephropathy patients showed significant differences in hemodynamics and postoperative maturity of autologous arteriovenous fistula.Accurately judging the mature state of autologous arteriovenous fistula in this regard has significant value for timely development of dialysis treatment and maintenance of smooth hemodialysis.
