自体富血小板血浆治疗糖尿病足的临床效果观察 被引量:2

摘要 目的探讨自体富血小板血浆治疗糖尿病足的临床效果。方法选择2020年7月—2021年9月该院收治的糖尿病足患者80例为研究对象。按照随机数表法分为两组,各40例。对照组实施清创换药,观察组于对照组基础上联合自体富血小板血浆治疗,比较两组治疗2周后肉芽组织覆盖率与生长厚度,创面疼痛缓解时间、结痂时间和愈合时间,比较两组治疗前后促创面生长因子,统计两组治疗期间发生不良反应的总比例。结果观察组治疗2周后肉芽组织覆盖率高于对照组,差异有统计学意义(P<0.05),肉芽组织生长厚度大于对照组,差异有统计学意义(P<0.05);观察组创面疼痛缓解时间、结痂时间和愈合时间均显著短于对照组,差异有统计学意义(P<0.05);干预后观察组晚期糖基化终末产物(AGEs)水平低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05),观察组血管内皮生长因子(VEGF)水平高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组发生创面疼痛、创面感染、创面颜色苍白和局部皮疹的总发生率显著低于对照组,差异有统计学意义(P<0.05)。结论自体富血小板血浆治疗糖尿病足,能有效的促进溃疡创面愈合,提高组织生长能力,缓解局部疼痛。 Objective To investigate the clinical effect of autologous platelet-rich plasma in the treatment of diabetic foot.Methods A total of 80 diabetic foot patients admitted to the hospital from July 2020 to September 2021 were selected as the research objects.According to the random number table method,they were divided into two groups,each with 40 cases.The control group was treated with debridement and dressing change,and the observation group was treated with autologous platelet-rich plasma on the basis of the control group.The granulation tissue coverage rate and growth thickness,wound pain relief time,scab formation time and healing time were compared between the two groups after 2 weeks of treatment.Wound growth-promoting factors were compared between the two groups before and after treatment.The total proportion of adverse reactions occurred in the two groups during the treatment period was counted.Results After 2 weeks of treatment,the coverage rate of granulation tissue in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05),and the growth thickness of the granulation tissue was greater than that in the control group,the difference was statistically significant(P<0.05);the pain relief time,scabbing time and healing time of the observation group were significantly shorter than those of the control group,the difference was statistically significant(P<0.05);after intervention,the level of advanced glycation end produces(AGEs)in the observation group was lower than before intervention,and the observation group was lower than the control group,the difference was statistically significant(P<0.05),the level of vascular endothelial growth factor(VEGF)was higher than the before intervention,and the observation group was higher than the control group,the difference was statistically significant(P<0.05);the total incidence of wound pain,wound infection,pale wound color and local rash in the observation group was significantly lower than that in the control group the difference was statistically significant(P<0.05).Conclusion Autologous platelet-rich plasma in the treatment of diabetic foot can effectively promote ulcer wound healing,improve tissue growth ability,and relieve local pain.
