右美托咪定对腰椎减压内固定术患者单核细胞⁃血小板黏附及单核细胞活化功能的影响

摘要 目的探究右美托咪定对腰椎减压内固定术患者单核细胞-血小板黏附及单核细胞活化功能的影响。方法选取2017年1月~2021年3月在本院行腰椎减压内固定术的患者60例,美国麻醉医师协会(ASA)分级为Ⅰ~Ⅱ级,采用随机数表法分为观察组(n=30)和对照组(n=30),给予观察组患者0.5μg/kg负荷量右美托咪定于麻醉诱导前10 min行注射,并在手术中与手术完成前15 min以0.5μg/(kg·h)速度维持泵注,给予对照组患者相同量生理盐水。比较手术前后相应时间点单核细胞-血小板黏附(CD42 a^(+)/CD14^(+))、单核细胞活化功能(HLADR^(+)/CD14^(+))。结果给予右美托咪定后,观察组CD42 a^(+)/CD14^(+)在T_(2)、T_(3)、T_(4)、T_(5)时间点明显较对照组低(P<0.05),且观察组CD42 a^(+)/CD14^(+)在各个时间点变化无明显差异(P>0.05);观察组HLADR^(+)/CD14^(+)在T_(4)、T_(5)时间点明显较对照组高(P<0.05)。结论腰椎手术患者手术过程应用右美托咪定可明显缓解炎性反应,并具调节免疫抑制功效。
