股骨近端髓内钉手术治疗老年骨质疏松性股骨转子间骨折的临床效果

Clinical effect of proximal femoral intramedullary nail in the treatment of elderly osteoporotic femoral intertrochanteric fractures

目的探讨不同股骨近端髓内钉手术治疗老年骨质疏松性股骨转子间骨折的临床效果。方法选取2019年1月至2021年6月成都市锦江区成都骨科医院创伤骨科的80例老年骨质疏松性股骨转子间骨折患者作为研究对象,采用计算机双色球分组法将其分为观察组和对照组,每组40例。观察组采用新一代股骨近端髓内钉(InterTAN)手术治疗,对照组采用股骨近端防旋髓内钉(PFNA)手术治疗。比较两组的手术治疗效果、患者恢复指标、手术并发症发生率。结果观察组的手术治疗总有效率高于对照组,差异有统计学意义(P<0.05);观察组的术后下地负重时间、骨折愈合时间短于对照组,差异有统计学意义(P<0.05);观察组的术后髋关节功能Harris评分高于对照组,差异有统计学意义(P<0.05);观察组的术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论针对老年骨质疏松性股骨转子间骨折患者的治疗,InterTAN手术治疗在恢复患者的关节功能、缩短患者的康复时间方面效果显著,且治疗安全性较高,值得临床应用。

Objective To investigate the clinical effect of different proximal femoral intramedullary nails in the treatment of osteoporotic femoral intertrochanteric fracture in the elderly.Methods A total of 80 elderly patients with osteoporotic femoral intertrochanteric fracture in the Department of Traumatology and Orthopedics of Chengdu Orthopedic Hospital of Jinjiang District of Chengdu City in Sichuan Province from January 2019 to June 2021 were selected as the research objects.They were divided into the observation group and the control group by computer two-color ball grouping method,with 40 patients in each group.The observation group was treated with interochanteric antegrade nai(interTAN),the control group was treated with proximal femoral nails antirotation(PFNA).The surgical treatment effect,patient recovery index and the incidence of surgical complications were compared between the two groups.Results The total effective rate of surgical treatment in the observation group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).The weight-bearing time and fracture healing time in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).The Harris score of hip function in the observation group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion For the treatment of elderly patients with osteoporotic femoral intertrochanteric fracture,interTAN operation has a significant effect in restoring the joint function and shortening the rehabilitation time of patients,and the treatment safety is higher,which is worthy of clinical application.
