经阴道超声联合宫腔镜检查对子宫内膜癌的诊断价值

Diagnostic value of transvaginal ultrasonography combined with hysteroscopy in endometrial cancer

目的探讨经阴道超声联合宫腔镜检查对子宫内膜癌的诊断价值。方法回顾性分析2019年1月至2021年4月厦门市中医院收治的93例经阴道超声检查发现子宫内膜异常并行宫腔镜检查的患者的临床资料,通过经阴道超声联合宫腔镜检查,以手术病理结果为金标准,比较不同诊断方式与金标准比较的敏感度、特异度、阳性预测值、阴性预测值及准确度。结果经阴道超声诊断子宫内膜癌8例,漏诊2例,误诊1例。经阴道超声+宫腔镜诊断子宫内膜癌9例,漏诊1例,误诊1例。经阴道超声联合宫腔镜检查的敏感度、阳性预测值、阴性预测值及准确度与经阴道超声比较,差异无统计学意义(P>0.05)。结论子宫内膜癌患者行经阴道超声检查与经阴道超声联合宫腔镜检查均可有效提高临床诊断率,可作为子宫内膜癌筛查及病情评估的方法,为子宫内膜癌的临床治疗提供有效参考。

Objective To explore the diagnostic value of transvaginal ultrasonography combined with hysteroscopy for endometrial cancer.Methods From January 2019 to the end of April 2021,93 patients with endometrial abnormalities by transvaginal ultrasonography and further performed with hysteroscopy who were admitted in Xiamen Hospital of Traditional Chinese Medicine were retrospectively selected as the research subjects.Transvaginal ultrasonography combined with hysteroscopy was performed using the surgical pathological results as the gold standard.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of different diagnostic methods were compared with gold standard.Results Endometrial carcinoma was diagnosed by transvaginal ultrasonography in 8 cases,missed diagnosis in 2 cases and misdiagnosis in 1 case.9 cases of endometrial carcinoma were diagnosed by transvaginal ultrasonography and hysteroscopy,1 case was missed and 1 case was misdiagnosed.There were no statistically significant differences in sensitivity,positive predictive value,negative predictive value and accuracy of transvaginal ultrasonography combined with hysteroscopy and transvaginal ultrasonography(P>0.05).Conclusion Transvaginal ultrasound and transvaginal ultrasonography combined with hysteroscopy in patients with endometrial cancer can toth effectively improve the clinical diagnosis rate,which can be used as a method for screening and disease evaluation of endometrial cancer,and provide an effective reference for clinical treatment of endometrial cancer.
