2016-2020年南通大学附属南通第三医院呼吸内科住院患者抗菌药物使用情况调查

Investigation on antimicrobial use of inpatients in Respiratory Department of Nantong Third Hospital Affiliated to Nantong University from 2016 to 2020

目的调查与分析2016—2020年南通大学附属南通第三医院呼吸内科住院患者抗菌药物的使用情况,进一步加强呼吸内科抗菌药物的合理使用。方法对照《全国抗菌药物临床应用专项整治活动方案》中提出的要求,通过合理用药监测系统(PASS)对2016—2020年呼吸内科住院患者抗菌药物使用量排名前5位、抗菌药物使用率、抗菌药物使用强度、接受抗菌药物治疗患者微生物样本送检率、抗菌药物的给药方式、抗菌药物联合使用情况分别进行统计及比较。结果2016—2020年碳青霉烯类抗菌药物的用量整体呈下降趋势,喹诺酮类抗菌药物中莫西沙星的用量呈下降趋势,三代头孢类抗菌药物的用量每年仍处于较高的用量,但青霉素、头孢菌素类加酶抑制剂的用量呈现逐年增加的趋势;呼吸内科住院患者抗菌药物的使用率呈下降趋势,从2016年的89.58%逐渐下降为2020年的72.11%,呼吸内科住院患者抗菌药物的使用强度随着使用率的下降也随之下降,由2016年的120.94下降为2020年的89.85;接受抗菌药物治疗患者总送检率、接受限制级抗菌药物治疗患者微生物样本送检率与接受特殊使用级抗菌药物治疗患者微生物样本送检率每年均符合标准,且2016—2020年总体呈上升趋势;呼吸内科住院患者的给药方式中以静脉给药占主要给药方式,比例高于口服给药与其他给药方式;呼吸内科住院患者单一用药的比例逐年升高,二联、三联及四联用药的比例整体呈下降趋势,但比例最高的仍是二联用药,2020年二联用药比例达54.67%,比例高于单一用药及其他联用情况。结论2016—2020年呼吸内科抗菌药物使用率、使用强度、送检率及联合使用抗菌药物等指标呈持续性改进。

Objective To investigate and analyze the use of antibiotics in hospitalized patients in Respiratory Department of Nantong Third Hospital Affiliated to Nantong University from 2016 to 2020,and further strengthen the rational use of antibiotics in respiratory department.Methods According to the requirements of the national special campaign plan for clinical application of antibacterial drugs,prescription automatic screening system(PASS)for rational use of antibiotics in hospital ranked the top 5 in the amount of antibacterial drugs used by inpatients in respiratory department of hospital from 2016 to 2020,the rate of antibacterial drugs used,the intensity of antibacterial drugs used,the rate of microbial samples submitted for examination of inpatients receiving antibacterial drugs,the way of antimicrobial drugs administered statistics and comparison were made on the combined use of antibiotics.Results From 2016 to 2020,the overall consumption of carbapenem antibiotics showed a downward trend,the consumption of Moxifloxacin in quinolone antibiotics decreased year by year,and the consumption of the third generation cephalosporin antibiotics was still at a high level every year,but the consumption of Penicillin and Cephalosporin Enzyme Inhibitors showed an increasing trend year by year.The use rate of antibiotics in inpatients of respiratory medicine decreased year by year,from 89.58%in 2016 to 72.11%in 2020.The use intensity of antibiotics in inpatients of respiratory department also decreased with the decrease of use rate,from 120.94 in 2016 to 89.85 in 2020.The total inspection rate of patients treated with antibiotics,the inspection rate of microbial samples of patients treated with restricted antibiotics and the inspection rate of microbial samples of patients treated with special use antibiotics met the standards every year,and there was an overall upward trend from 2016 to 2020.Intravenous administration was the main mode of administration in hospitalized patients in respiratory department,which was higher than oral administration and other modes of administration.The proportion of single drug use in hospitalized patients in respiratory department was increasing year by year,and the proportion of double,triple and quadruple drugs showed a downward trend as a whole,but the highest proportion was still double drug use.In 2020,the proportion of double drug use had reach 54.67%,which was higher than that of single drug use and other combinations.Conclusion From 2016 to 2020,the utilization rate,use intensity,submission rate and combined use of antibiotics in respiratory department showed continuous improvement.
