肾康合剂治疗脾肾两虚夹痰瘀型糖尿病肾病临床观察

Efficacy observation of Shenkang mixture on diabetic nephropathy with syndrome of spleen-kidney both deficiency with phlegm and blood-stasis

目的:观察肾康合剂治疗脾肾两虚夹痰瘀型糖尿病肾病Ⅲ期患者的临床疗效。方法:选择符合糖尿病肾病Ⅲ期纳入标准患者60例,随机分为两组各30例,两组均予西医常规疗法,包括降糖、降压等治疗,治疗组予肾康合剂治疗,对照组予坎地沙坦片治疗。3个月后观察两组的临床疗效,比较两组治疗前后症状积分及FBG、2hPPG、HbA1c水平的变化,以及对尿微量白蛋白(MALB)、尿微量白蛋白/尿肌酐比值(MALB/Cr)的影响。结果:治疗组治疗后FBG、2hPPG、HbA1c、MALB、MALB/Cr水平较治疗前降低(P﹤0.01);与对照组比较,MALB、MALB/Cr降低更明显(P﹤0.05)。治疗后两组症状积分均下降(P﹤0.01),且治疗组总有效率76.67%显著优于对照组53.33%(P﹤0.05)。两组均无毒副反应。结论:肾康合剂治疗脾肾两虚夹瘀型糖尿病肾病Ⅲ期患者,可有效改善其临床症状、体征,提高生活质量。

Objective:To observe the clinical efficacy of Shenkang mixture on the 3rd stage of diabetic nephropathy with syndrome of spleen-kidney both deficiency with phlegm and blood-stasis.Methods:60 cases of selected patients with the 3rd stage of diabetic nephropathy,who met the inclusion criteria,were randomly divided into two groups,30 cases in each group.Both groups were treated with conventional western medicine including hypoglycemic,antihypertensive and other treatments.On the basis of this,the treatment group was treated with Shenkang mixture,while the control group was treated with candesartan tablets.After 3 months of treatment,the clinical efficacy was observed in two groups.The changes of symptom score and levels of FBG,2hPPG and HbA1c as well as the effects on urinary microalbumin(MALB)and urinary microalbumin/urine creatinine ratio were compared between two groups before and after treatment.Results:The levels of FBG,2hPPG,HbA1c,MALB and MALB/Cr were decreased in treatment group after treatment compared with before treatment(P﹤0.01);compared with the control group after treatment,MALB and MALB/Cr of treatment group were decreased more significantly(P﹤0.05).After treatment,the symptom score was decreased in both groups(P﹤0.01);the total effective rate of treatment group was 76.67%,which was significantly better than 53.33%of control group(P﹤0.05).There were no toxic and side effects in both groups.Conclusion:Shenkang mixture for treating the 3rd stage of diabetic nephropathy with syndrome of spleenkidney both deficiency with phlegm and blood-stasis can effectively improve the clinical symptoms and physical signs of patients,and enhance the quality of life.
