关节镜辅助复位内固定与切开复位内固定治疗胫骨平台骨折的疗效比较 被引量:2

Comparison of Curative Effect between Arthroscopic Assisted Reduction and Internal Fixation and Open Reduction and Internal Fixation in the Treatment of Tibial Plateau Fractures

摘要 目的 探讨对胫骨平台骨折患者分别采用关节镜辅助复位内固定方法及切开复位内固定方法完成治疗后获得临床效果。方法 方便选取该院2017年1月—2020年12月收治的52例胫骨平台骨折患者作为研究对象,随机分为常规组(采用切开复位内固定方法治疗)和研究组(采用关节镜辅助复位内固定方法治疗),各26例,比较两组患者治疗优良率、膝关节功能评分、手术相关指标及并发症总发生率。结果 研究组治疗优良率(96.15%)高于常规组(65.38%),差异有统计学意义(χ^(2)=7.924,P<0.05);经治疗后,研究组膝关节活动度、HSS评分均高于常规组,术中失血量、骨折愈合时间、切口长度均优于常规组,差异有统计学意义(P<0.05);研究组并发症总发生率(11.54%)低于常规组,差异有统计学意义(P<0.05)。结论 关节镜辅助复位内固定方法有效应用后,同切开复位内固定治疗效果比较,可显著提高患者治疗效果,改善膝关节功能,改善手术指标,减少并发症,可促进胫骨平台骨折患者总体预后水平显著提升。 Objective To investigate the clinical effects of arthroscopic assisted reduction and internal fixation and open reduction and internal fixation in patients with tibial plateau fractures.Methods 52 patients with tibial plateau fractures admitted to the hospital from January 2017 to December 2020 were conveniently selected as the research objects.They were randomly divided into routine group(treated by open reduction and internal fixation) and study group(treated by arthroscopic-assisted reduction and internal fixation),with 26 cases in each group.The excellent and good rate of treatment,knee joint function score,surgery-related indicators and the total incidence of complications were compared between the two groups.Results The excellent and good rate of treatment in the study group(96.15%) was higher than that in the routine group(65.38%),and the difference was statistically significant (χ^(2)=7.924,P<0.05).After treatment,the knee range of motion and HSS score in the study group were higher than those in the routine group.Intraoperative blood loss,fracture healing time,and incision length were better than those in the routine group,and the difference between the two groups was statistically significant(P<0.05).The total incidence of complications in the study group(11.54%) was lower than that in the routine group,and the difference was statistically significant(P<0.05).Conclusion After the effective application of arthroscopic-assisted reduction and internal fixation,compared with the treatment effect of open reduction and internal fixation,the treatment effect of patients can be significantly improved,and the function of the knee joint can be improved,surgical indicators and complications can significantly be improved,as well as the overall prognosis of patients with tibial plateau fractures.
