基于FMEA模型的渐进性康复护理在股骨颈骨折术后患者中的应用

Effects of progressive rehabilitation nursing based on FMEA in patients after femoral neck fracture

目的探讨基于失效模式与效应分析(FMEA)模型的渐进性康复护理在股骨颈骨折术后患者中的应用效果。方法采用便利抽样法,选取于2017年12月1日—2020年12月31日龙游县人民医院骨科接诊的股骨颈骨折患者为研究对象。本研究共纳入302例患者,采用随机数字表法分为观察组(n=150)和对照组(n=152),对照组患者采用常规康复护理,观察组患者在对照组基础上给予基于FMEA模型的渐进性康复护理。比较两组患者护理前、护理2个月时的髋关节评分量表、医院膝关节评分量表(HSS)的评分,并比较两组患者关节优良率、术后并发症发生率。结果护理2个月时,观察组患者的髋关节评分量表评分、HSS评分、关节功能优良率均高于对照组,差异均有统计学意义(P<0.05);观察组患者骨不连、粘连严重、骨折延迟愈合的总发生率低于对照组,差异有统计学意义(P<0.05)。结论股骨颈骨折患者在术后采用FMEA模型进行渐进式康复护理,可以提高患者髋膝关节功能的恢复,提高关节功能优良率,值得在临床中推广。

Objective To explore the effect of progressive rehabilitation nursing based on failure mode and effect analysis(FMEA)in patients after femoral neck fracture.Methods From December 1,2017 to December 31,2020,convenience sampling was used to select patients with femoral neck fractures admitted to the Department of Orthopedics of Longyou County People's Hospital as the research subject.A total of 302 patients were included and divided into the observation group(n=150)and the control group(n=152)by random number table method.The patients in the control group were given routine rehabilitation nursing,and the patients in the observation group received the progressive rehabilitation nursing based on FMEA on the basis of the control group.The scores of the Harris Hip Score(HHS)and Hospital Knee Score Scale(HSS)were compared between the two groups before nursing and two months after nursing,and the excellent and good rate of joint function and incidence of postoperative complications were compared between the two groups.Results After two months of nursing,the HHS score,HSS score,and excellent and good rate of joint function in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of nonunion,serious adhesion and delayed fracture union in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions The application of progressive rehabilitation nursing based on FMEA in patients after femoral neck fracture can improve the recovery of hip and knee joint function and the rate of excellent and good joint function,which is worthy of promotion in clinical practice.
