摘要
目的分析我国分娩镇痛试点医院的组织保障现状,为进一步推进分娩镇痛工作相关政策提供客观依据。方法收集912家分娩镇痛试点医院的政策保障与试点组织实施情况、医院内分娩镇痛管理服务人才队伍配置情况、医护人员绩效分配与激励政策支持情况、分娩镇痛工作专项收费情况4方面资料,采用定性与定量相结合的方法分析试点医院组织保障的经验和不足。结果存在麻醉科和产科医护人员短缺、分娩镇痛工作绩效分配不明确、分娩镇痛收费标准尚未确立、分娩镇痛未纳入医保报销等问题。结论建议探索分娩镇痛转岗培训,加强分娩镇痛人才队伍建设,建立产科多学科协作团队;建立合理的绩效分配政策,调动相关科室医务人员的积极性;明确分娩镇痛收费标准,落实医保报销,减轻孕产妇经济负担。
Objective To analyze the organizational security status of labor pain pilot hospitals in China,and to provide an objective basis for further promoting labor analgesia related policies.Method The data of 912 labor analgesia pilot hospitals were collected from four aspects:policy guarantee and implementation of the pilot organization,allocation of labor analgesia management and service personnel in the hospital,performance distribution and incentive policy support of medical staff,and special charges for labor analgesia.The experience and deficiency of the pilot organization guarantee were analyzed by combining qualitative and quantitative methods.Result There were some problems,such as the shortage of anesthesiologists and obstetricians,the unclear performance distribution of labor analgesia,the undetermined charging standard of labor analgesia,and the labor analgesia was not included in the medical insurance reimbursement.Conclusion It is suggested to improve the performance allocation mechanism of pilot hospitals to effectively mobilize the enthusiasm of medical staff in relevant departments,clarify the charging standards for labor analgesia,implement medical insurance reimbursement,and reduce the economic burden of pregnant and lying-in women.
