双歧杆菌三联活菌散治疗腹型过敏性紫癜患儿疗效及血清S-ChE、LPO及sTLR2水平的影响 被引量:5

摘要 目的探讨双歧杆菌三联活菌散治疗腹型过敏性紫癜患儿疗效及血清血清胆碱酯酶(S-ChE)、过氧化脂质(LPO)及可溶性Toll样受体2(sTLR2)水平的影响。方法前瞻性选取2018年8月至2020年8月唐山市开滦总医院收治的115例腹型过敏性紫癜患儿作为研究对象,按照随机数字表法分为两组。其中对照组58例,采用抗组胺等常规疗法;观察组57例,在对照组基础上联用双歧杆菌三联活菌散治疗,温水冲服治疗,3次/d,1.0 g/次,两组均连续使用2周。观察并记录两组患儿的临床疗效及皮疹、腹痛、紫癜、血尿及便血等症状消退时间、治疗前后血清S-ChE、LPO及sTLR2水平。结果对照组脱落3例,共纳入55例,观察组脱落2例,共纳入55例。观察组临床总有效率为92.73%,显著高于对照组(76.36%),差异有统计意义(P<0.05)。观察组患儿皮疹、腹痛、紫癜、血尿及便血等症状消退时间为(5.10±1.15)、(3.02±0.85)、(5.21±1.57)、(5.37±1.87)、(2.93±0.99)d,均显著短于对照组[(8.19±1.60)、(4.19±1.03)、(7.62±2.08)、(7.73±2.02)、(4.28±1.15)d],差异均有统计意义(P<0.05)。两组治疗后血清S-ChE和sTLR2水平均明显升高,血清LPO水平明显下降,且观察组治疗后血清S-ChE及sTLR2水平为(4.31±0.91)kU/L、(9.85±1.93)pg/mL,高于对照组[(3.19±0.84)kU/L、(7.75±2.02)pg/mL],血清LPO水平为(8.93±2.01)μmol/mL,低于对照组[(10.07±2.44)μmol/mL],差异均有统计意义(P<0.05)。两组腹胀、恶心、红肿、瘙痒、皮疹等不良反应发生情况比较,差异无统计学意义(P>0.05)。结论双歧杆菌三联活菌散可提高儿童腹型过敏性紫癜的S-ChE和sTLR2水平,降低LPO水平,疗效明显,缩短症状消退时间,值得推广。 Objective To investigate the therapeutic effect of bifidobacterium triplex activating powder on children with abdominal allergic purpura and the influence of serum cholinesterase(S-ChE),lipid peroxide(LPO)and soluble Toll-like receptor 2(sTLR2)levels.Methods A total of 115 children with abdominal hypersensitivity purpura admitted to Kailuan General Hospital in Tangshan City from August 2018 to August 2020 were prospectively selected as the research objects,and divided into two groups according to random number table method.Among them,58 cases of control group were treated with the conventional therapies such as antihistamines,57 cases of observation group were treated with bifidobacterium triple viable powder on the basis of the control group,water a blunt treatment,3 times/d,1.0 g/time,both groups were used for 2 consecutive weeks.The clinical efficacy,resolving time of symptoms such as rash,abdominal pain,purpura,hematuria and hematochezia,and serum S-CHE,LPO and sTLR2 levels before and after treatment were observed and recorded.Results There were 3 cases of detachment in the control group and 2 cases of detachment in the observation group.The total clinical effective rate of the observation group was 92.73%,which was significantly higher than that of the control group(76.36%),and the difference was statistically significant(P<0.05).The regression time of rash,abdominal pain,purpura,hematuria and hematozoia in the observation group were(5.10±1.15)d,(3.02±0.85)d,(5.21±1.57)d,(5.37±1.87)d,(2.93±0.99)d,which were significantly shorter than those in the control group[(8.19±1.60)d,(4.19±1.03)d,(7.62±2.08)d,(7.73±2.02)d,(4.28±1.15)d],and the differences were statistically significant(P<0.05).After treatment,the levels of serum S-CHE and sTLR2 in 2 groups were significantly increased,and the level of serum LPO was significantly decreased,the levels of serum S-ChE and sTLR2 in the observation group after treatment were(4.31±0.91)kU/L,(9.85±1.93)pg/mL,which were higher than those in the control group[(3.19±0.84)kU/L,(7.75±2.02)pg/mL],the serum LPO level was(8.93±2.01)μmol/mL,which was lower than that of the control group[(10.07±2.44)μmol/mL],and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions such as abdominal distention,nausea,redness,itching,and rash between the two groups(P>0.05).Conclusion Bifidobacterium triplex viable powder can improve s-CHE and sTLR2 levels and reduce LPO levels in children with abdominal allergic purpura,with obvious curative effect and shortening the time of symptom resolution,which is worthy of promotion.
