锁定钢板内固定术与交锁髓内钉内固定术治疗胫腓骨骨折的效果研究

The Effect of Locking Plate Internal Fixation and Interlocking Intramedullary Nailing in the Treatment of Tibia and Fibula Fractures

目的 比较锁定钢板(locking compress plate,LCP)与交锁髓内钉(interlocking intramedullarnail,IIN)内固定术治疗胫腓骨骨折的效果。方法 方便选取2018年6月—2021年5月新泰市人民医院收治的胫腓骨骨折患者62例,为研究对象采用随机数表法分为两组,各31例。对照组行LCP内固定术,研究组行IIN内固定术。比较两组手术时间、术中出血量、住院时间、骨折愈合时间,及踝关节功能恢复的优良率与术后并发症情况。结果研究组手术时间(78.23±8.12)min、术中出血量(105.65±12.03)mL较对照组(62.52±7.50)min、(87.30±14.03)m L高,住院时间(14.02±2.03)d、骨折愈合时间(2.25±0.65)个月较对照组(18.00±2.52)d、(3.00±0.45)个月短,差异有统计学意义(t=7.913、5.528、6.848、5.282,P<0.05)。研究组踝关节功能恢复的优良率96.77%高于对照组74.19%,差异有统计学意义(χ^(2)=4.679,P<0.05)。术后并发症发生率比较,研究组3.23%与对照组9.68%差异无统计学意义(χ^(2)=0.267,P>0.05)。结论 相较于LCP,IIN治疗胫腓骨骨折的效果更为理想,且缩短了术后康复速度,适于临床推广。

Objective To compare the effect of locking plate(LCP)and interlocking intramedullary nail(IIN)in the treatment of tibia and fibula fractures.Methods It is convenient to select 62 patients with tibia and fibula fractures admitted to Xintai City People's Hospital from June 2018 to May 2021 as the research objects.They were divided into two groups by random number table,wih 31 cases in each group.The control group received LCP internal fixation,and the study group received IIN internal fixation.The operation time,intraoperative blood loss,hospital stay,fracture healing time,and the excellent and good rate of ankle function recovery and postoperative complications were compared between the two groups.Results Compared with the control group(62.52±7.50)min and(87.30±14.03)mL,the operation time(78.23±8.12)min and intraoperative blood loss(105.65±12.03)mL of the study group were higher,hospital stay(14.02±2.03)d and fracture healing time(2.25±0.65)months were shorter than control group(18.00±2.52)d,(3.00±0.45)months,and the difference was statistically significant(t=7.913,5.528,6.848,5.282,P<0.05).The excellent and good rate of ankle function recovery in the study group was 96.77%higher than that in the control group 74.19%,and the difference was statistically significant(χ^(2)=4.679,P<0.05).In the comparison of postoperative complication rates,there was no statistically significant difference between 3.23%in the study group and 9.68%in the control group(χ^(2)=0.267,P>0.05).Conclusion Compared with LCP,IIN has a more ideal effect in the treatment of tibia and fibula fractures,and shortens the postoperative recovery speed,which is suitable for clinical promotion.
