多层螺旋CT对甲状腺癌的诊断价值 被引量:3

摘要 目的探究多层螺旋CT(MSCT)对甲状腺癌的诊断价值。方法将80例甲状腺结节患者依据病理学诊断结果分为恶性组(n=42,甲状腺癌)及良性组(n=38,甲状腺良性结节)。对比两组患者MSCT影像学特征、CT值,分析MSCT对甲状腺癌的诊断价值。结果恶性组患者结节形态不规则率、边缘模糊率、不均匀强化率、囊实性及实性率、砂砾样钙化率均明显高于良性组,差异均有统计学意义(P﹤0.01);两组患者结节直径比较,差异无统计学意义(P﹥0.05)。恶性组患者增强与平扫CT值的差值明显高于良性组,差异有统计学意义(P﹤0.01)。MSCT诊断甲状腺癌的灵敏度为88.10%,特异度为81.58%,准确度为85.00%。结论甲状腺良恶性结节的MSCT影像学特征除结节直径外,差异均较大,诊断结果可为临床的早期诊断及进一步治疗提供参考依据,具有较高的应用价值。 Objective To explore the diagnosis value of multi-slice spiral CT(MSCT)in the thyroid cancer.Method A total of 80 patients with thyroid nodules were divided into malignant group(n=42,thyroid cancer)and benign group(n=38,benign thyroid nodules)based on the pathological results;the MSCT imaging features,CT value were compared be-tween the two groups,and the diagnosis value of MSCT in the thyroid cancer was analyzed.Result The morphology ir-regular rate,edge blurred rate,uneven enhancement rate,cystic consolidation and solid rate,gravel-like calcification rate in malignant group were higher than those in benign group(P<0.01);there was no significant difference in diameter of nodules between two groups(P>0.05).The differential value of enhanced and plain CT in the malignant group was higher than that in the benign group(P<0.01).The sensitivity,specificity and accuracy of MSCT in the diagnosis of thyroid can-cer were 88.10%,81.58%and 85.00%,respectively.Conclusion There are significant differences for the MSCT imag-ing features in diagnosing malignant and benign thyroid nodules except for the diameter of nodules.The diagnosis results can provide a reference for early clinical diagnosis and further treatment,which have high application value.
