血液标志物在卵圆孔未闭相关隐源性卒中中的表达特点和临床价值

The expression characteristics and clinical value of blood markers in patent foramen ovale related cryptic stroke

目的 观察血液性标志物在隐源性卒中患者中的表达特点,探讨其临床应用价值。方法 以卵圆孔未闭作为关键词收集患者病历进行回顾性分析,其中依据TOAST分型标准确诊的隐源性卒中患者129例(CS组),缺血性脑卒中患者75例(对照组),比较两组患者一般临床资料、凝血指标及炎症相关指标水平。结果 CS组肾小球滤过率、白细胞、中性粒细胞、血小板/淋巴细胞以及中性粒细胞/淋巴细胞水平高于对照组,高密度脂蛋白水平低于对照组,差异均具有统计学意义(P<0.05);通过绘制ROC曲线,仅血小板/淋巴细胞、中性粒细胞/淋巴细胞为CS的预测因子(P<0.05),当血小板/淋巴细胞为113.83、中性粒细胞/淋巴细胞为1.99时的敏感性和特异性最佳,曲线下面积分别为0.583、0.600。结论 血小板/淋巴细胞、中性粒细胞/淋巴细胞对预测卵圆孔未闭隐源性卒中有一定的临床价值。

Objective To observe the expression characteristics of blood markers in patients with cryptic stroke and discuss clinical application value.Methods The medical records of patients with patent foramen ovale were collected for retrospective analysis.There were 129patients with cryptic stroke confirmed according to TOAST classification criteria(CS group)and 75patients without ischemic stroke(control group),then compared the general clinical data,blood coagulation indexes and inflammation related indexes between the two groups.Results The levels of glomerular filtration rate,leukocytes,neutrophils,platelets/lymphocytes and neutrophils/lymphocytes in the CS group were higher than those in the control group,and the levels of high-density lipoprotein were lower than those in the control group,with statistically significant differences(P<0.05).By drawing ROC curve,platelet/lymphocyte ratio and neutrophil/lymphocyte ratio were the only predictors of CS(P<0.05).When platelet/lymphocyte ratio was 113.83and neutrophil/lymphocyte was 1.99,the sensitivity and specificity were the best,and the area under the curve was respectively 0.583and 0.600.Conclusion Platelet/lymphocyte ratio and neutrophil/lymphocyte ratio had certain clinical value in predicting cryptic stroke of patent foramen otal.
