摘要
目的 分析新型冠状病毒流行期间南京市儿童医院就诊患儿呼吸道病原学检测结果,并与去年同期检测结果进行对比观察。方法 对1053例于2020年1月至3月在南京市儿童医院就诊的呼吸道疾病患儿痰标本呼吸道腺病毒(AdV)、甲型流感病毒(FluA)、乙型流感病毒(FluB)、副流感Ⅰ型病毒(PIV1)、副流感Ⅱ型病毒(PIV2)、副流感Ⅲ型病毒(PIV3)和呼吸道合胞病毒(RSV)等7项病原体抗原检测结果进行回顾性分析。此外,对1099例呼吸道感染患儿外周血血清中Q热立克次体(COX-IgM)、肺炎衣原体(CPa-IgM)、甲型流感病毒(INFA-IgM)、乙型流感病毒(INFB-IgM)、嗜肺军团菌血清1型(LP1-IgM)、肺炎支原体(MP-IgM)、副流感Ⅰ、Ⅱ、Ⅲ型病毒(PIV1PIV2PIV3-IgM)、呼吸道合胞病毒(RSV-IgM)和腺病毒(ADV-IgM)等9项呼吸道病原体IgM结果进行回顾性分析。同时,对2019年同一时间段就诊的1888例呼吸道感染患儿痰7项病原体抗原检测结果以及3609例呼吸道感染患儿血清9项病原体IgM进行回顾性分析。结果 2020年1-3月期间,在1053例呼吸道疾病患儿痰标本中,阳性例数241例,阴性例数812例,阳性患儿以RSV感染最为常见,其中检测出2种病毒合并感染13例。1099例呼吸道感染患儿血清中病原体IgM检测结果显示,阳性病例数232例,阴性病例数867例,以MP-IgM阳性多见,2种及以上抗体联合检出共计14例,以MP和其他病原体抗体联合检出为主。2019年1-3月期间,在1888例呼吸道疾病患儿痰标本中,阳性例数566例,阴性例数1322例,阳性患儿以RSV感染最为常见,其中检测出2种病毒合并感染18例。3609例呼吸道感染患儿血清中病原体IgM检测结果显示,阳性病例数1058例,阴性病例数2551例,以MP-IgM阳性多见,2种及以上抗体联合检出共计228例,以MP和其他病原体抗体联合检出为主。结论 新型冠状病毒流行期间南京市儿童医院就诊呼吸道感染患儿痰标本检测病毒抗原以RSV最为常见,血液标本检测IgM类抗体以MP多见,与2019年同期基本一致,本研究对临床进行本地区呼吸系统疾病预判具有指导意义。
Objective Analysis of respiratory pathogen detection results of children with respiratory disease in Nanjing Children’s Hospital during the outbreak of COVID-19,and the results were compared with those of the same period in 2019.Methods The pathogen antigen from sputum specimens of 1053children with respiratory diseases in Nanjing Children’s Hospital from Janauary to March in 2020were analyzed.The pathogen IgM from serum specimen of1099children with respiratory diseases during the same period were also analyzed.Besides,the pathogen antigen from sputum specimens of 1888children and the pathogen IgM from serum specimen of 3609children with respiratory diseases from Janauary to March in 2019were also analyzed.Results In the sputum samples of 1053children with respiratory diseases from January to March in 2020,241cases were positive and 812cases were negative.RSV infection was the most common infection in the positive cases.13cases of the positive cases were co-infected with 2viruses.In the serum sample of 1099children with respiratory diseases,232cases were positive and 867cases were negative.MP-IgM was the most commom dectcted in the positive cases.A total of 14cases were detected with 2or more antibodies.In the period of January to March in 2019,1888cases(566positive cases and 1322negative cases)of sputum samples and 3609cases(1058positive cases and 2551negative cases)of serum sample from children with respiratory diseases were also analyzed.As the same period in 2020,RSV and MP-IgM were the most commom dectcted in the positive cases.Conclusion RSV was the most common virus antigen detected in sputum samples of children with respiratory tract infection in Nanjing Children’s Hospital during the novel coronavirus epidemic period,and MP was the most common IgM antibody detected in blood samples.These results is basically consistent with those in the same period in 2019.This study had guiding significance for clinical prediction of respiratory diseases in the region.
