高速涡轮钻联合超声骨刀在拔出下颌水平阻生智齿中的应用效果

Effect of High-Speed Turbo Drill Combined with Ultrasonic Bone Knife in Extraction of Mandibular Horizontal Obstructed Wisdom Teeth

目的研究高速涡轮钻联合超声骨刀在下颌水平阻生智齿拔出中的效果。方法选择2017年8月至2019年8月联勤保障部队第九九〇医院收治的86例(86颗)下颌水平阻生智齿患者,随机分为联合组(43例,高速涡轮钻联合超声骨刀)与常规组(43例,高速涡轮钻拔除)。比较两组手术指标(术中出血量、手术时间)、术后恢复指标(疼痛程度、张口受限程度及面部肿胀程度)、术后并发症发生情况及术后生活质量。结果联合组术中出血量较常规组少,手术时间较常规组短,差异有统计学意义(P<0.05);联合组术后疼痛程度、张口受限程度、面部肿胀程度等级较常规组低,差异有统计学意义(P<0.05);两组术后并发症发生率比较差异无统计学意义(P>0.05);联合组术后外观、疼痛评分均小于常规组,差异有统计学意义(P<0.05);联合组与常规组术后饮食、语言、感觉、恶心呕吐、日常活动得分及总分比较,差异无统计学意义(P>0.05)。结论高速涡轮钻联合超声骨刀在拔出下颌水平阻生智齿中的应用效果较好,术后恢复程度较好,并发症发生率低,值得应用。

Objective To study the effect of combined extraction of mandibular horizontal obstructive wisdom teeth with high-speed turbo drill and ultrasonic bone knife.Methods A total of 86 patients with mandibular horizontal obstructive wisdom teeth admitted to the Ninety Nine Hundred Hospital of the Joint Logistics and Security Forces from August 2017 to August 2019 were selected and randomLy divided into combined group(43 patients)and conventional group(43 patients).In the conventional group,the teeth were extracted using a high-speed turbine drill,and in the combined group,the teeth were extracted using a high-speed turbine drill combined with an ultrasonic bone knife.The surgical indexes(intraoperative bleeding,operative time),postoperative recovery indexes(degree of pain,degree of mouth opening restriction and degree of facial swelling),occurrence of postoperative complications and postoperative quality of life were compared between the 2 groups.Results The combined group had less intraoperative bleeding and shorter operative time than the conventional group,and all differences were statistically significant(P<0.05).The degree of postoperative pain,degree of mouth opening restriction,and grade of facial swelling in the combined group were lower than those in the conventional group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications between the 2 groups(P>0.05).The postoperative appearance and pain scores of the combined group were lower than those of the conventional group,and the differences were statistically significant(P<0.05).There were no statistically significant differences between the postoperative diet,speech,sensation,nausea and vomiting,daily activity scores and total scores of the combined group and the conventional group(P>0.05).Conclusion The application of high-speed turbo drill combined with ultrasonic bone knife in extraction of mandibular horizontal obstructed wisdom teeth is worthy of application because of the low intraoperative bleeding,short operation time,better postoperative recovery degree and low complication rate.
