摘要
目的探讨新生儿肺炎(neonatal pneumonia)患者接受无创呼吸机辅助治疗的临床疗效.方法选取天津市静海区医院新生儿科治疗的58例新生儿肺炎患儿,并分为观察组和对照组各29例.对照组患儿通过经鼻导管或面罩方式吸氧,氧浓度30%-40%.观察组患儿在常规治疗基础上,使用无创呼吸机治疗,工作模式设定为S/T,氧流量2-8L/min,吸入氧浓度0.3-0.5,吸气压力3-5cmH2O,气道压力15-22cmH2O,通气时间5-15h/d.治疗期间对患儿生命体征进行密切监测,根据其病情变化调整呼吸机参数.比较两组患者临床疗效、症状改善或消失时间、住院时间有无差异.结果观察组患儿气促改善、湿啰音消失、鼻阻消失以及住院时间均明显短于对照组,差异有统计学意义(P<0.05).观察组患儿的总有效率为96.55%,对照组为79.31%,差异明显且有统计学意义(P<0.05).结论对新生儿肺炎患儿应用无创呼吸机治疗可促进其症状改善或消失,缩短住院时间,提高临床总体疗效,应用效果更佳.
Objective To investigate the elinical efcacy of non-invasive venilator in neonates with pneumonia.Methods A tolal of 58 cases of neonates with pneumonia in the neonatology department of Jinghai District Hospital of Tianjin were selected and divided into observation group and control group,29 eases in each.The control group received oxygen through nasal catheter or mask.oxygen concentration of 30%to 40%.0n the basis of conventional trealment,children in the observation group were treated with non-invasive ventilator.The working mode was set as S/T,oxygen flow 2-81/min,inbaled oxygen concentration 0.3-0.5.inspiratory pressure 3-5emH_(2)O,respiratory pressure 15-22cmH_(2)0,and ventilation time 5-15h/d.During the treatment,the vital signs of the children were closely monitored,and ventilator parameters were adjusted according to the changes of their condition.The improvement or disappearance time of elinical symptoms and length of hospital stay were compared between the two groups.Results The improvement of shortness of breath,disappearance of wet rlesdisappearance of nasal obstruction and hospitalization time in the observation group were signifcantly shorter than those in the control group.with stistical signifcance(P<0.05),.The total efctive rate was 96.55%in the observation group and 79.31%in the control group,and the lfference was saistically signifcant(P<0.05).Conc lusion The application of non-invasive ventilator in neonates with pneumonia can improve or disappear the symptoms,shorten the hospital stay,improve the overall elinical efficacy,and the application ffect is better.
