社区多元化护理模式在老年高血压患者院外管理中的应用

Application of Community Diversified Nursing Model in Out-of-hospital Management of Elderly Patients with Hypertension

目的探讨社区多元化护理模式在老年高血压社区管理中的作用。方法收集2018—2021年于青岛市黄岛区隐珠街道东楼路社区卫生服务中心建档的老年高血压患者60例,随机分为对照组和观察组各30例。对照组采用常规社区护理模式,观察组采用社区多元化护理模式。比较两组患者干预后血压改善情况、自我护理能力和自我效能。结果两组患者干预后收缩压、舒张压、脉压差异有统计学意义(P<0.05);两组患者干预后自我护理测定量表各条目及总分差异有统计学意义(P<0.05);两组患者干预后一般自我效能感量表评分分布差异有统计学意义(P<0.05)。结论社区多元化护理更有针对性,能够进一步强化老年高血压患者的血压控制,并提高自我护理能力和改善自我效能,值得社区卫生服务机构推广应用。

Objective To explore the role of community diversified nursing model in community management of elderly hypertension.Methods A total of 60 elderly patients with hypertension filed in Donglou Road community health service center,Yinzhu street,Huangdao District,Qingdao,from 2018 to 2021 were randomly divided into control group and observation group.The control group adopted the conventional community nursing model,and the observation group adopted the community diversified nursing model.The improvement of blood pressure,selfcare ability and self-efficacy were compared between the two groups.Results There were significant differences in systolic blood pressure,diastolic blood pressure and pulse pressure between the two groups(P<0.05).There were significant differences in the items and total scores of self-care agency scale between the two groups after intervention(P<0.05).There was significant difference in the score distribution of general self-efficacy scale between the two groups after intervention(P<0.05).Conclusion Community diversified nursing is more targeted,which can further strengthen the blood pressure control of elderly patients with hypertension,improve self-care ability and improve selfefficacy.It is worthy to be popularized and applied by community health service institutions.
