Clinical value of N-terminal pro-brain natriuretic peptide and C-reactive protein detection in the prediction of acute cerebral infarction with cerebral heart syndrome

摘要 目的探讨N-末端脑钠肽前体(NT-proBNP)、C反应蛋白(CRP)检测在预测急性脑梗死脑心综合征中的临床价值。方法选取2018年2月至2020年12月本院收治的80例急性脑梗死患者作为研究对象,按照有无脑心综合征分为未发组和已发组,每组40例。两组均进行NT-proBNP、CRP水平及心肌酶谱检测和超声心电图检查,分析NT-proBNP及CRP与心功能指标的相关性,分析NT-proBNP、CRP检测对急性脑梗死脑心综合征的预测效能。结果已发组NT-proBNP、CRP、肌酸激酶同工酶(CK-MB)及心肌肌钙蛋白T(cTnT)水平均高于未发组,且左心室射血分数(LVEF)水平低于未发组,差异有统计学意义(P<0.05)。经Pearson相关性分析显示,NT-proBNP、CRP水平与LVEF水平呈负相关,与cTnT、CK-MB水平呈正相关(P<0.05)。经工作特性曲线(ROC)分析,患者NT-proBNP、CRP水平预测急性脑梗死脑心综合征的曲线下面积(AUC)分别为0.862、0.829,预测灵敏度分别为89.41%、81.30%。结论NT-proBNP、CRP检测能较好地预测急性脑梗死脑心综合征,预测价值较高,临床应密切监测患者的CRP、NT-proBNP水平,以及时、准确地判断患者脑损伤情况,为后续临床治疗提供参考依据。 Objective To investigate the clinical value of N-terminal pro-brain natriuretic peptide(NT-proBNP)and C-reactive protein(CRP)in the prediction of acute cerebral infarction with cerebral heart syndrome.Methods A total of 80 patients with acute cerebral infarction admitted to our hospital from February 2018 to December 2020 were selected as the research subjects,and they were divided into non developed group and developed group according to the presence or absence of cerebro cardiac syndrome,with 40 cases in each group.The levels of NT-proBNP and CRP,Myocardial Zymogram and ultrasonic electrocardiogram were detected in both groups,the correlation between NT-proBNP,CRP and cardiac function indexes was analyzed,and the predictive efficacy of NT-proBNP and CRP on acute cerebral infarction with cerebral heart syndrome was analyzed.Results The levels of NT-proBNP,CRP,creatine kinase isoenzyme(CK-MB)and cardiac troponin T(cTnT)in the developed group were higher than those in the non developed group,and the levels of left ventricular ejection fraction(LVEF)was lower than that in the non developed group,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that the levels of NT-proBNP and CRP were negatively correlated with LVEF levels,and positively correlated with cTnT and CK-MB levels(P<0.05).According to the analysis of working characteristic curve(ROC),the area under the curve(AUC)predicted by the levels of NT-proBNP and CRP were 0.862 and 0.829 respectively,and the prediction sensitivity was 89.41%and 81.30%respectively.Conclusion The detection of NT-proBNP and CRP can better predict the occurrence of acute cerebral infarction with cerebral heart syndrome,and the prediction value is high.The levels of CRP and NT-proBNP in patients should be closely monitored in clinic,so as to judge the brain injury of patients in time and accurately,and provide reference basis for follow-up clinical treatment.
