声门下吸引染色测量人工气道气囊压力的应用研究

摘要 目的探讨声门下吸引染色测量人工气道气囊压力的应用效果。方法60例需持续人工气道气囊压力机械通气的患者,随机分为研究组和对照组,各30例。对照组采用PORTEX专用气囊压力监测表测量气囊压力,研究组采用声门下吸引染色测量气囊压力。比较两组患者气管插管拔管时间、气管切开堵管时间及呼吸机相关性肺炎(VAP)发生率。结果研究组患者气管插管拔管时间(15.35±10.65)d和气管切开堵管时间(4.00±1.50)d均显著短于对照组的(28.25±20.75)、(7.00±1.30)d,差异具有统计学意义(P<0.05)。研究组患者VAP发生率为6.67%,低于对照组的26.67%,差异具有统计学意义(P<0.05)。结论使用声门下吸引染色测量人工气道气囊压力,可有效缩短气管插管拔管时间和气管切开堵管时间,VAP发生率明显降低,临床可积极应用。 Objective To discuss the application effect of subglottic aspiration staining in measuring the pressure of artificial airway gasbag.Methods A total of 60 patients requiring continuous artificial airway pressure mechanical ventilation were randomly divided into the research group and the control group,with 30 cases in each group.The control group was measured by PORTEX special pressure monitoring meter,and the research group was measured by subglottic aspiration staining.The time of tracheal extubation and tracheotomy,and the incidence of ventilator-associated pneumonia(VAP)were compared between the two groups.Results The time of tracheal extubation and tracheotomy in the research group were(15.35±10.65)and(4.00±1.50)d,which were significantly shorter than(28.25±20.75)and(7.00±1.30)d in the control group,and the differences statistically significant(P<0.05).The incidence of VAP in the research group was 6.67%,which was lower than 26.67%in the control group,and the difference was statistically significant(P<0.05).Conclusion Using subglottic aspiration staining to measure the pressure of artificial airway gasbag can effectively shorten the time of tracheal extubation and tracheotomy,and lower the incidence of VAP,which can be actively used in clinical practice.
