不同手术方式治疗结肠癌合并肠梗阻的效果比较

Comparison of the effects of different surgical methods in the treatment of colon cancer with intestinal obstruction

目的比较不同手术方式治疗结肠癌合并肠梗阻的效果。方法选取78例结肠癌合并肠梗阻患者作为研究对象,按照随机数字表法分为研究组和对照组,每组39例。对照组患者进行常规结肠次全切除术治疗,研究组患者实施左半结肠切除一期吻合术治疗。对比两组患者并发症发生情况、手术出血量、手术时长以及住院天数。结果研究组患者的并发症发生率为5.1%(2/39),显著低于对照组的20.5%(8/39),差异具有统计学意义(P<0.05)。研究组患者的手术出血量(159.09±39.21)ml少于对照组的(249.23±42.67)ml,手术时长(134.59±21.32)min以及住院天数(13.49±1.23)d均短于对照组的(220.34±25.56)min、(18.90±2.43)d,差异具有统计学意义(P<0.05)。结论临床中对结肠癌合并肠梗阻患者实施左半结肠切除一期吻合术治疗,能够在一定程度上降低患者并发症发生几率,且手术各项指标较传统手术更理想,具有更高的实用性,临床中可根据患者情况及意愿选择合适手术。

Objective To compare the effects of different surgical methods in the treatment of colon cancer with intestinal obstruction.Methods A total of 78 patients with colon cancer and intestinal obstruction were selected as the research subjects,and were divided into research group and control group according to the random numerical table,with 39 cases in each group.Patients in the control group were treated with conventional subtotal colectomy,and the patients in the research group were treated with left hemicolectomy with one-stage anastomosis.The complication rate,surgical blood loss,operation time and hospitalization time were compared between the two groups.Results The complication rate in the research group was 5.1%(2/39),which was significantly lower than 20.5%(8/39)in the control group,and the difference was statistically significant(P<0.05).The surgical blood loss(159.09±39.21)ml in the research group was less than(249.23±42.67)ml in the control group,and the operation time(134.59±21.32)min and hospitalization time(13.49±1.23)d were shorter than(220.34±25.56)min and(18.90±2.43)d in the control group.All the differences were statistically significant(P<0.05).Conclusion For patients with colon cancer and intestinal obstruction,the left-side hemicolectomy with one-stage anastomosis can reduce the probability of complication rate in the patient to a certain extent,and it is better than traditional surgery in various surgical indicators.It is more ideal,has higher practicability,and is suitable for clinical application according to patients'conditions and wishes.
