摘要
目的比较间歇性蓝光照射和连续性蓝光照射治疗新生儿黄疸的临床效果。方法162例新生儿黄疸患儿,依据随机数字表法分为A组和B组,每组81例。A组患儿接受连续性蓝光照射治疗,B组采用间歇性蓝光照射治疗。比较两组患儿的治疗效果、不良反应发生率以及治疗前后血清总胆红素、粪胆红素水平。结果B组治疗总有效率93.83%与A组的92.59%比较差异无统计学意义(P>0.05)。B组不良反应发生率为8.64%,显著低于A组的23.46%,差异有统计学意义(P<0.05)。治疗前,两组血清总胆红素、粪胆红素水平比较差异无统计学意义(P>0.05);治疗2 d,两组血清总胆红素水平均低于治疗前,粪胆红素水平均高于治疗前,差异有统计学意义(P<0.05),B组血清总胆红素、粪胆红素水平与A组比较,差异无统计学意义(P>0.05)。结论在新生儿黄疸治疗中应用间歇性蓝光照射治疗和连续性蓝光照射治疗均能取得显著的疗效,但间歇性蓝光照射治疗对新生儿造成的不适更少,新生儿不良反应发生率更低,故而更推荐临床大力应用间歇性蓝光照射治疗。
Objective To compare the clinical effect of intermittent blue light irradiation in the treatment of neonatal jaundice.Methods A total of 162 cases of neonatal jaundice were divided into group A and group B according to the random numerical table,with 81 cases in each group.Group A received continuous blue light irradiation,and group B received intermittent blue light irradiation.The therapeutic effect,incidence of adverse reactions,and serum total bilirubin and fecal bilirubin levels before and after treatment were compared between the two groups.Results There was no statistically significant difference in the total effective rate of 93.83%in group B and 92.59%in group A(P>0.05).The incidence of adverse reactions in group B was 8.64%,which was significantly lower than 23.46%in group A,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in serum total bilirubin and fecal bilirubin levels between the two groups(P>0.05).After 2 d of treatment,the levels of serum total bilirubin in the two groups were lower than those before treatment in this group,and fecal bilirubin were higher than those before treatment in this group.All the differences were statistically significant(P<0.05).There was no statistically significant difference in serum total bilirubin and fecal bilirubin levels between group B and group A(P>0.05).Conclusion In the treatment of neonatal jaundice,both intermittent blue light irradiation therapy and continuous blue light irradiation therapy can achieve significant efficacy,but intermittent blue light irradiation causes less discomfort to the neonate and has a lower incidence of adverse neonatal reactions,so it is more recommended that intermittent blue light irradiation be used clinically.
