摘要 目的探讨多学科协作(multi-disciplinary team,MDT)管理模式在多重耐药菌感染防控中的效果。方法采用回顾性研究,调查医院2019年1—11月实施多重耐药菌多学科协作模式前与2020年1—11月实施多学科协作模式后住院患者目标监测的多重耐药菌的检出率、抗菌药物使用情况等。结果实施MDT前2019年1—11月共检出多重耐药菌207株(8.19%),实施MDT后2020年1—11月共检出多重耐药菌160株(6.50%),实施MDT后多重耐药菌的检出率及抗菌药物使用率均较前明显下降,差异有统计学意义(P<0.05);MDT实施后抗菌药物使用强度29.61明显低于实施前的41.84。医护人员的手卫生、微生物的送检率、接触隔离的执行情况均高于实施前。结论MDT管理模式能有效降低医院多重耐药菌的感染,提高抗菌药物使用的合理性,降低抗菌药物的使用强度。 Objective To explore the effect of multi-disciplinary team(MDT)management mode in the prevention and control of multidrugresistant organism infection.Methods A retrospective study was used.To investigate the detection rate of multidrug-resistant organism and the use of antibiotics for target monitoring of inpatients before the implementation of the multi-disciplinary collaboration model for multidrug-resistant organism in the hospital from January to November 2019 and after the implementation of the multi-disciplinary collaboration model from January to November 2020.Results Before the implementation of MDT,a total of 207 strains(8.19%)of multidrug-resistant organism were detected from January to November in 2019.After the implementation of MDT,a total of 160 strains of multidrug-resistant organism(6.50%)were detected from January to November in 2020.The detection rate of multidrug-resistant organism and the use rate of antibiotics were significantly lower than before,and the difference was statistically significant(P<0.05).After the implementation of MDT,the intensity of antimicrobial use of 29.61 was significantly lower than that before the implementation of 41.84.The hand hygiene of medical staff,the submission rate of microorganisms,and the implementation of contact isolation were all higher than those before the implementation.Conclusion The MDT management model can effectively reduce the infection of multidrug-resistant organism in hospitals,improve the rationality of the use of antibiotics,and reduce the antimicrobial use density.
