全专联合管理模式在社区房颤患者中的应用及对管理质量的影响

Application of general practice and specialty joint management mode in community patients with atrial fibrillation and the impact on management quality

目的:探讨全专联合管理模式在社区房颤患者中的应用及对管理质量的影响。方法:选取2019年7月—2021年7月上海市普陀区长风街道白玉社区卫生服务中心收治的房颤患者84例,采用随机数字表法分为两组,每组42例。对照组采用常规管理模式干预,观察组采用全专联合管理模式干预。干预后,对患者管理效果进行评估,比较两组心率、血压、自我管理评分变化、满意度及依从性。结果:干预后,两组心率、收缩压及舒张压均低于干预前(P<0.05),且观察组低于对照组(P<0.05);干预后,观察组药物管理、临床症状管理、饮食管理以及社会适应管理评分均高于对照组(P<0.05);观察组干预后护理方法、服务态度、护患沟通满意度和定期复查、遵医饮食、按时用药依从性高于对照组(P<0.05)。结论:临床采取全专联合管理模式对社区房颤患者进行干预,能够稳定患者心率和血压,有效改善患者生活习惯,提高疾病认知度,增加自我管理能力,值得推广应用。

Objective:To explore the application of the general practice and specialty joint management mode in community patients with atrial fibrillation and the impact on the quality of management.Methods:A total of 84 patients with atrial fibrillation admitted to Baiyu Community Health Service Center,Changfeng Sub District,Putuo District,Shanghai from July 2019 to July 2021 were selected and divided into two groups with 42 cases in each group by random number table method.The control group was intervened by conventional management mode,and the observation group was intervened in the general practice and specialty joint management mode.After intervention,the management effect of patients was evaluated,and the changes in heart rate,blood pressure,self-management scores,satisfaction and compliance were compared between the two groups.Results:After the intervention,the heart rate,systolic blood pressure and diastolic blood pressure of the two groups were lower than those before the intervention(P<0.05),and the observation group was lower than the control group(P<0.05);after intervention,the scores of drug management,clinical symptom management,diet management and social adaptation management in the observation group were higher than those in the control group(P<0.05);the nursing methods,service attitude,nurse-patient communication satisfaction,regular review,medical diet,and on-time medication compliance in the observation group were higher than those in the control group after the intervention(P<0.05).Conclusion:Intervention in community atrial fibrillation patients with the general practice and specialty joint management mode can stabilize the patient’s heart rate and blood pressure,effectively improve the patients’living habits,improve the disease awareness,and increase the self-management ability,which is worthy of popularization and application.
