围手术期优质护理对妊娠期高血压疾病产妇产后出血的影响

Effect of Perioperative Quality Nursing on Postpartum Hemorrhage of Pregnant Women with Hypertension

目的探究妊娠期高血压疾病产妇行围手术期优质护理的可行性。方法随机将2020年9月至2021年9月我科收治的74例妊娠期高血压疾病产妇分为试验组(37例,应用优质护理)、对照组(37例,应用常规护理)。对比两组护理满意度、产后出血率、术中出血量、产后2 h出血量、干预前后血压变化、生存质量评分、SAS评分、SDS评分。结果试验组总满意度(97.30%,36/37)高于对照组(81.08%,30/37),P<0.05;试验组产后出血率(5.41%,2/37)低于对照组(24.32%,9/37),P<0.05;试验组术中出血量(170.38±20.67)mL、产后2 h出血量(284.29±26.44)mL,与对照组的(215.52±33.26)mL、(325.54±34.48)mL相比,P<0.05;经干预,试验组(134.28±10.18)mm Hg、舒张压(90.11±8.21)mm Hg均低于对照组,P<0.05;干预后,试验组躯体疼痛评分(86.47±10.21)分、精神状况评分(87.28±12.17)分、情感功能评分(85.79±11.38)分、总体健康评分(85.33±12.07)分均高于对照组,P<0.05;经干预,试验组SAS评分(25.68±2.05)分、SDS评分(22.34±2.07)分均低于对照组,P<0.05;经干预,试验组术后不良反应发生率显著低于对照组,P<0.05。结论在围手术期合理运用优质护理对于妊娠期高血压疾病产妇产后出血的影响显著,可降低产后出血发生率,临床效果显著。

Objective To explore the feasibility of perioperative high-quality nursing for pregnant women with gestational hypertension.Methods A total of 74 pregnant women with gestational hypertension in our department from September 2020 to September 2021 were randomly divided into experimental group(37 cases,using high-quality nursing)and control group(37 cases,using routine nursing).The nursing satisfaction,postpartum hemorrhage rate,intraoperative bleeding volume,postpartum 2 h bleeding volume,blood pressure changes before and after intervention,quality of life score,SAS score and SDS score were compared between the two groups.Results The total satisfaction of the experimental group(97.30%,36/37)was higher than that of the control group(81.08%,30/37)(P<0.05).The rate of postpartum hemorrhage in the experimental group(5.41%,2/37)was lower than that in the control group(24.32%,9/37)(P<0.05).The amount of intraoperative bleeding(170.38±20.67)mL and 2-hour postpartum bleeding(284.29±26.44)mL in the experimental group were compared with(215.52±33.26)mL,(325.54±34.48)mL in the control group(P<0.05).After intervention,the experimental group(134.28±10.18)mm Hg and diastolic blood pressure(90.11±8.21)mm Hg were lower than those in the control group(P<0.05).The physical pain score(86.47±10.21),the mental status score(87.28±12.17),the emotional function score(85.79±11.38)and the overall health score(85.33±12.07)in the experimental group were higher than those in the control group(P<0.05).After intervention,the SAS score(25.68±2.05)and SDS score(22.34±2.07)of the experimental group were lower than those of the control group(P<0.05);After intervention,the incidence of postoperative adverse reactions in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion The rational use of high-quality nursing during perioperative period has a significant impact on postpartum hemorrhage of pregnant women with hypertension,and can reduce the incidence of postpartum hemorrhage.The clinical effect is significant.
