心理干预联合营养支持对腹膜透析终末期肾病患者生活质量的影响

Effect of Psychological Intervention Combined with Nutritional Support on Quality of Life of Patients with End-stage Renal Disease on Peritoneal Dialysis

目的研究心理干预联合营养支持对腹膜透析终末期肾病(ESRD)患者生活质量的影响。方法选择2020年5月至2021年5月我院收治的84例腹膜透析治疗的ESRD患者,根据随机数字表法分为对照组和研究组,各42例。两组均予以心理干预,观察组联合营养支持干预,对比干预前后的生活质量评分和营养指标(血清蛋白、前清蛋白、血红蛋白)、C反应蛋白(CPR)以及白细胞介素-6(IL-6)含量,比较护理前后两组患者心理状态改善情况[汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)]。结果护理前,两组前清蛋白、血清蛋白、血红蛋白、CPR、IL-6水平对比,差异无统计学意义(P>0.05);护理后,研究组血清蛋白、血清蛋白、血红蛋白水平均高于对照组,CPR和IL-6水平低于对照组(P<0.05)。研究组在护理后的各项生活质量评分均高于对照组(P<0.05)。研究组在护理后的HAMA和HAMD评分均低于对照组(P<0.05)。结论针对腹膜透析治疗的ESRD患者实施心理干预联合营养支持干预,可提高其生活质量,缓解患者的营养不良状态,改善其负性心理状态。

Objective To study the effect of psychological intervention combined with nutritional support on the quality of life of patients with end-stage renal disease(ESRD)on peritoneal dialysis.Methods From May 2020 to May 2021,84 patients with ESRD treated by peritoneal dialysis in our hospital were selected and divided into the control group and the study group with 42 cases in each group according to the random number table method.Both groups were given psychological intervention.After that,the observation group was combined with nutritional support intervention.The quality of life scores and nutritional indicators(serum protein,prealbumin,hemoglobin),C-reactive protein(CPR)and interleukin-6(IL-6)content before and after the intervention were compared.The improvement of mental state of patients in the two groups before and after[Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)].Results Before nursing,there was no difference in prealbumin,serum protein,hemoglobin,CPR and IL-6 between the two groups(P>0.05);after nursing,the above indicators in the study group were higher than those in the control group,CPR and IL-6 lower than the control group(P<0.05).The quality of life scores in the study group were higher than those in the control group(P<0.05).The HAMA and HAMD scores of the study group after nursing were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Psychological intervention combined with nutritional support intervention in ESRD patients treated by peritoneal dialysis can improve the quality of life,alleviate the malnutrition state and improve the negative psychological state.
