快速康复外科理念在胃肠道术围手术期护理中的临床应用效果

Rapid Rehabilitation Surgery Lies in the Clinical Effect of Perioperative Nursing in Gastrointestinal Surgery

目的研究快速康复外科理念在胃肠道手术患者围手术期护理中的应用效果。方法于我院胃肠道手术患者中随机抽取206例,分为对照组(103例,常规护理)和观察组(103例,快速康复外科护理),对比两组的护理质量。结果观察组拔除胃管时间、排气时间、排便时间分别为(40.21±3.50)、(36.24±3.27)、(42.74±5.16)h,显著短于对照组的(45.21±2.64)、(40.59±3.12)、(48.30±5.96)h,差异有统计学意义(P<0.05)。观察组下床活动时间、住院时间、术后24 h CRP分别为(1.23±0.41)d、(6.80±1.35)d、(42.78±1.23)mg/L,较对照组的(1.94±0.51)d、(8.92±1.15)d、(65.58±1.12)mg/L明显减少,差异有统计学意义(P<0.05)。治疗后,观察组SF-36评分、SAS评分分别为(75.42±4.63)、(41.73±3.90)分,显著优于对照组的(69.24±4.80)、(46.39±4.23)分,差异有统计学意义(P<0.05)。观察组总并发症发生率为1.94%,较对照组的7.77%显著降低,差异有统计学意义(P<0.05)。结论针对胃肠道手术患者给予围手术期快速康复外科护理,可有效缩短患者的术后康复时间,提高临床康复效率,有利于改善患者的生活质量,并改善其不良情绪,对于预防并发症有积极的影响,具有较高的临床应用价值。

Objective To study the application of rapid rehabilitation surgery in perioperative nursing of patients undergoing gastrointestinal surgery.Methods A total of 206 patients with gastrointestinal surgery in our hospital were randomly divided into control group(103 cases,routine nursing)and observation group(103 cases,rapid rehabilitation surgical Nursing).The nursing quality of the two groups was compared.Results In the observation group,the time of removing gastric tube(40.21±3.50)h,the time of exhausting gas(36.24±3.27)h and the time of defecating(42.74±5.16)h were significantly shorter than those in the control group[the time of removing gastric tube(45.21±2.64)h,the time of exhausting gas(40.59±3.12)h and the time of defecating(48.30±5.96)h],the difference was statistically significant(P<0.05).The ambulatory time(1.23±0.41)d,hospital stay(6.80±1.35)d and postoperative 24 h CRP(42.78±1.23)mg/L in the observation group were significantly lower than those in the control group[ambulatory time(1.94±0.51)d,hospital stay(8.92±1.15)d,postoperative 24 h CRP(65.58±1.12)mg/L],the difference was statistically significant(P<0.05).After treatment,the scores of SF-36(75.42±4.63)and SAS(41.73±3.90)in the observation group were significantly better than those in the control group[SF-36(69.24±4.80)and SAS(46.39±4.23)],the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was 1.94%,which was significantly lower than 7.77% in the control group,and the difference was statistically significant(P<0.05).Conclusion Perioperative rapid rehabilitation surgical nursing for patients undergoing gastrointestinal surgery can effectively shorten the postoperative rehabilitation time,improve the clinical rehabilitation efficiency,improve the quality of life of patients and improve their bad mood.It has a positive impact on the prevention of complications and has high clinical value.
